Abstract

La hipertensió portal és la síndrome clínica caracteritzada per un increment patològic de la pressió hidrostàtica del sistema venós portal per sobre del seu valor normal que oscil•la entre 1-5 mmHg. Aquest increment origina la formació d’una extensa xarxa de venes col•laterals porto sistèmiques que deriven part del flux sanguini portal cap a la circulació sistèmica, evitant el seu pas pel fetge. Bona part de les complicacions derivades de la cirrosi hepàtica, com l’hemorràgia digestiva causada pel trencament de varius esofàgiques, l’encefalopatia hepàtica, l’ascites o la insuficiència renal, estan directament relacionades amb la presència d’hipertensió portal clínicament significativa, definida per un gradient de pressió portal mínim de 10mmHg. Donada la gravetat d’aquestes complicacions, la hipertensió portal representa la primera causa de mort i transplantament hepàtic en malalts cirròtics (1). La hipertensió portal pot aparèixer per un augment de la resistència vascular intrahepàtica, per un augment del flux portal o bé, per un augment d’ambdós factors. Tot i això, l’augmenta de la resistència vascular intrahepàtica és el factor fisiopatològic primari del manteniment i desenvolupament de la hipertensió portal. Per tant, tractaments dirigits a reduïr la resistència vascular intrahepàtica produiran també una disminució en la pressió portal reduint així el risc de patir complicacions. Actualment la principal teràpia dels malalts cirròtics és l’ús d’agents β-bloquejants que, redueixen la pressió portal disminuint el flux venós portal que comporta una descens en el consum cardíac i una vasoconstricció esplànica (2-4). En els darrers anys s’ha demostrat que és possible manipular la resistència vascular intrahepàtica farmacològicament (sense necessitat de modificar el flux portal)(5, 6). Els fetges cirròtics presenten un marcat desequilibri entre les substàncies vasoactives encarregades de la regulació del to vascular hepàtic. A més, produeixen una hiperesposta als estímuls vasoconstrictors i una hiporesposta als estímuls vasodilatadors , els quals com és el cas de l’òxid nítric es troben en baixes concentracions. Actualment encara es desconeix una gran part de mediadors vasoactius involucrats en l’augment de la resistència vascular intrahepàtica o bé aquells mecanismes que regulen la seva acció. Fins l’actualitat s’han postulat diferents mecanismes per demostrar la reduïda biodisponibilitat d’òxid nítric com les regulacions post-traduccionals que pateix l’enzim eNOS o bé el seu cofactor: BH4 (7, 8). L’elevat estrés oxidatiu que pateixen els fetges cirròtics podria ser també una causa de la baixa biodisponibilitat d’òxid nítric (9). Per això els organismes han desenvolupat uns sistemes antioxidants que inhibeixen de forma directa o indirecta els efectes nocius de les espècies reactives d’oxigen (ROS). Un exemple és l’enzim SOD encarregat de metabolitzar el radical superòxid en peroxinitrit. Malauradament, en la cirrosi trobem la seva activitat disminuïda (BASIC). El Tempol com a mimètic de l’activitat de l’enzim SOD ha demostrat augmentar la biodisponibilitat d’òxid nítric, disminuint l’estrès oxidatiu i la pressió portal de rates cirrotiques amb hipertensió portal. No obstant, ha produït una disminució en la pressió arterial sistèmica indicant que el seu espectre d’acció no és exclusivament intrahepàtic. En els darrers anys l’estudi dels polifenols presents a la dieta ha generat un gran interès per les seves propietats antioxidants, antimutagèniques, anticarcinogèniques i antiinflamatòries (10). Un dels més estudiats és la isoforma trans del Resveratrol (3,5,4’-trihydroxystilbè) que ha demostrat disminuir l’estrés oxidatiu, augmentar la biodisponibilitat d’òxid nítric i disminuir la pressió portal de rates cirròtiques amb hipertensió portal provocant la regressió del procés fibròtic i una millora en el fenotip endotelial. Tot i que encara no es pot parlar d’un únic fàrmac per curar la malaltia, els treballs englobats en la present tesi doctoral i, els que han estat publicats paral•lelament suggereixen que el tractament adient pels malalts cirròtics podria ser els agents β-bloquejants actualment utilitzats acompanyats d’una dieta sana i equilibrada rica en agents antioxidants. Increased hepatic vascular resistance to portal blood flow is the main cause of portal hypertension in cirrhosis and is the result of the structural changes inherent to cirrhosis and an increased hepatic vascular tone (1). Insufficient nitric oxide (NO) availability in the liver circulation is considered as a major pathogenic factor increasing hepatic vascular tone in cirrhosis(2, 3), which has been attributed to a decreased endothelial NO synthase (eNOS) activity due to alterations in its posttranslational regulation(4, 5). In addition, scavenging of NO by increased levels of superoxide (O2−) is another factor that has been shown to reduce NO bioavailability(6, 7). In cirrhotic livers there is an increased O2− production due to both an increased O2− release by enzymatic systems, such as cyclooxygenase (COX) and/or xanthine oxidase (XO), and a reduced expression and activity of superoxide dismutase (SOD), a critical enzyme that metabolises O2− (8, 9). The increased O2− levels found in cirrhotic livers, further contribute to reduce NO bioavailability within the liver by scavenging NO, in a reaction in which O2− reacts with NO to form peroxynitrite (ONOO−) (8). Supporting such a role for oxidative stress in reducing hepatic NO availability in patients with cirrhosis, the acute administration of high doses of the antioxidant vitamin C reduces MDA levels and improves intrahepatic endothelial dysfunction (10). Tempol (4-hydroxy-2,2,6,6-tetramethylpiperdine-N-oxyl) is a potent antioxidant of low molecular weight that permeates biological membranes, mimics SOD activity, and scavenges O2− anions in vitro and in vivo (11). These studies have demonstrated that Tempol reduces superoxide, increases nitric oxide, and reduces portal pressure in sinusoidal endothelial cells and in cirrhotic livers. These results confirm that oxidative stress has a role in the pathogenesis of portal hypertension and supports the use of antioxidants in its treatment. However, when considering the use of antioxidants as additional therapy to treat portal hypertension, the potential to produce deleterious effects on systemic hemodynamics needs to be carefully evaluated. Resveratrol, a polyphenol found in a variety of fruits, exerts a wide range of beneficial effects on the endothelium, regulates multiple vasoactive substances and decreases oxidative stress, factors involved in the pathophysiology of portal hypertension. Our studies confirm that chronic Resveratrol administration reduces portal pressure, hepatic stellate cell activation and liver fibrosis, and improves hepatic endothelial dysfunction in cirrhotic rats, suggesting it may be a useful dietary supplement in the treatment of portal hypertension in patients with cirrhosis.
