Abstract

Introducción: El ictus supone una de las principales causas de mortalidad y morbilidad en los países desarrollados. En el caso del ictus isquémico, las diversas etiologías y mecanismos fisiopatológicos se asocian a diferencias en su pronóstico y tasa de recurrencia, lo que determina que las estrategias para la prevención secundaria sean diferentes. Uno de los sistemas más aceptados para definir el mecanismo del ictus es la clasificación aplicada en el Trial of Org 10172 in Acute Ischemic Stroke (TOAST). Empleando datos clínicos y paraclínicos, clasifica el mecanismo del ictus en patología ateroesclerótica de gran vaso, embolismo cardiaco, patología de vasos pequeños, etiologías indeterminadas, y otras causas. La clasificación no emplea marcadores bioquímicos y los datos que pueden sugerir una relación entre éstos y el mecanismo del ictus son escasos. Por ello, el objetivo principal del presente estudio fue determinar los niveles plasmáticos de marcadores de trombosis (D-dímero, fibrinógeno, cociente D-dímero/fibrinógeno), e inflamación (proteína C reactiva -PCR- y velocidad de sedimentación globular -VSG-), durante la fase aguda del ictus isquémico para estudiar diferencias entre pacientes y controles, y entre los diferentes subtipos de ictus. Secundariamente, se estudió la relación entre estos parámetros y otras variables analíticas y clínicas adicionales. Pacientes y métodos: Estudio clínico, prospectivo y controlado, que incluyó 200 pacientes ingresados consecutivamente en la Unidad de Ictus del Hospital Pêro da Covilhã con diagnóstico de ictus isquémico (50 de cada subgrupo TOAST, excluyendo la categoría de otras causas) de volución inferior a 24 horas, y 50 controles. Se registraron las variables demográficas, antecedentes patológicos, factores de riesgo vascular, gravedad clínica (National Institutes of Health Stroke Scale, NIHSS), y clasificación clínica del ictus (Oxfordshire Community Stroke Project). Las variables bioquímicas principales (D-dímero, fibrinógeno, VSG, PCR, y cociente D-dímero/fibrinógeno) y secundarias (hemograma, glucemia, perfil lipídico, proteinograma) fueron analizadas en dos muestras diferentes de sangre venosa durante las primeras 48 horas. Se registraron las complicaciones neurológicas y sistémicas, la situación funcional al alta (escala modificada de Rankin, definiendo dependencia como su valor superior a 2), y el probable mecanismo del ictus (criterios TOAST). Resultados: Cincuenta y cinco pacientes presentaron un síndrome TACI, 56 PACI, 71 LACI, y 18 POCI. El síndrome TACI se asoció a valores superiores de D-dímero y del cociente D-dímero/fibrinógeno. Considerando los mecanismos, la mediana de VSG fue mayor en los pacientes con ictus aterotrombótico y lacunar. Los pacientes con ictus cardioembólico y los controles presentaron los menores valores, sin que se observaran diferencias entre subtipos. La mediana de fibrinógeno fue significativamente superior en todos los subtipos de ictus comparados con los controles. El nivel fue superior en el subtipo cardioembólico, pero las diferencias entre subgrupos no fueron significativas. Las medianas de D-dímero y del cociente D-dímero/fibrinógeno fueron superiores en los pacientes con ictus cardioembólico (p<0.0001 al comparar ambos parámetros con los controles y pacientes con ictus aterotrombótico y lacunar; p=0.002 al comparar el D-dímero, y p=0.004 al comparar el cociente con los valores de los pacientes con ictus indeterminado). La mediana de PCR fue significativamente superior en todos los grupos de ictus respecto a los controles. Al comparar los pacientes entre sí, la mediana de PCR fue significativamente superior en los pacientes con ictus aterotrombótico respecto a los pacientes con ictus lacunar (p=0.010), e indeterminado (p=0.003). Se definieron tres perfiles bioquímicos: (1) inflamatorio; (2) protrombótico; (3) mixto. El inflamatorio fue observado en los pacientes con ictus aterotrombótico y fue definido básicamente por el aumento de PCR, y, secundariamente, por el de los valores de fibrinógeno y VSG. El protrombótico fue observado en los pacientes con ictus cardioembólico y se caracterizó por el aumento de D-dímero y del cociente, y, secundariamente, por el aumento del fibrinógeno, y por la ausencia de incremento de PCR y VSG. El perfil mixto fue observado en los pacientes con diagnóstico de ictus lacunar y de mecanismo indeterminado, y presentó características bioquímicas intermedias. Al alta, 81 pacientes (40.5%) fueron independientes. Las variables relacionadas con la dependencia fueron la edad (OR 1.050, IC 95% 1.010-1.092, p=0.014) y la NIHSS (OR 1.576, IC 95% 1.332-1.865, p<0.0001). Setenta y siete pacientes (30.8%) experimentaron complicaciones durante el ingreso. Las variables relacionadas independientemente con las complicaciones infecciosas fueron la edad (OR 1.084, IC 95% 1.033-1.138, p=0.001) y la NIHSS (OR 1.343, IC 95% 1.205-1.495, p<0.0001). Las relacionadas con la progresión del ictus fueron el antecedente de claudicación intermitente (OR 12.604, IC 95% 2.297-69.144, p=0.004) y la NIHSS (OR 1.156, IC 95% 1.049-1.273, p=0.003). Siete pacientes (3.5%) fallecieron. La mortalidad se asoció a la NIHSS (OR 1.197, IC 95% 1.041-1.376, p=0.012). Discusión: Los resultados mostraron la presencia de alteraciones analíticas durante las primeras 72 horas de la fase aguda del ictus isquémico. Comparados con los controles, los pacientes presentaron niveles superiores de fibrinógeno, D-dímero, cociente D-dímero/fibrinógeno, VSG, PCR, leucocitos, alfa-1 y alfa-2 globulinas, y menores de albúmina. Dado que los pacientes con complicaciones sistémicas graves fueron excluidos del estudio, las diferencias halladas se deben básicamente al ictus, representando la respuesta de fase aguda y los procesos asociados a sus causas. Excepto el D-dímero, la ausencia de correlación entre las variables principales de laboratorio y los síndromes TACI sugiere que las diferencias observadas se asocian en parte a las causas del ictus. Los pacientes con ictus cardioembólico presentaron un perfil trombogénico compatible con la activación permanente del sistema de la coagulación-fibrinólisis, mecanismo apoyado por los elevados valores del cociente D-dímero/fibrinógeno. Los pacientes con ictus aterotrombótico presentaron un perfil inflamatorio que sugiere la participación de procesos inflamatorios sistémicos y de la disfunción endotelial. En los pacientes con ictus lacunar se observó un perfil mixto con predominio de la inflamación. En los pacientes con ictus indeterminado se observó un perfil mixto con predominio trombogénico que sugiriere que en parte de estos pacientes el mecanismo del ictus puede ser cardioembólico. Las asociaciones halladas entre la dependencia y la edad y la NIHSS, entre la incidencia de complicaciones infecciosas y la edad, entre el desarrollo de ictus progresivo y la NIHSS, y entre la mortalidad y la NIHSS, son coherentes con los resultados de estudios publicados previamente y confirman el valor pronóstico de la evaluación clínica inicial de los pacientes con ictus.


Introduction: Stroke is one of the main causes of mortality and disability in developed countries. Considering ischemic stroke, its different aetiologies and mechanisms are related to differences in prognosis and risk of recurrence. This determines capital differences in strategies of secondary prevention. The classification applied in the Trial of Org 10172 in Acute Ischemic Stroke (TOAST) is one of the more used systems to define the mechanism of stroke. Using clinical and paraclinical data, it classifies subtypes of stroke in large vessel atherosclerotic disease, cardiac embolism, small vessel disease, undetermined aetiology, and other causes. The classification does not use biochemical markers and there are few data to support an association between the biomarkers and the mechanism of stroke. Then, the main aim of the present study was to determine plasmatic levels of thrombosis markers (D-dimer, fibrinogen, D-dimer/fibrinogen rate), and inflammation (C reactive protein -CRP- and erythrocyte sedimentation rate -ESR-), during acute phase of ischemic stroke, to assess the differences between patients and controls, and among subtypes of stroke. Secondarily, the relationship between these variables and additional analytic and clinical parameters was assessed. Patients and methods: This was a clinical study, prospective and controlled, including 200 patients consecutively admitted to Stroke Unit of Hospital Pêro da Covilhã with diagnosis of less than 24 hours ischemic stroke (50 of each TOAST subtype excluding the category of other aetiologies), and 50 controls. Demographic variables, pathological antecedents, vascular risk factors, clinical severity (National Institutes of Health Stroke Scale, NIHSS), and clinical classification of stroke (Oxfordshire Community Stroke Project) were recorded. The main (D-dimer, fibrinogen, ESR, CRP, and D-dimer/fibrinogen rate) and secondary (haemogram, glycaemia, lipid profile, proteinogram) biochemical variables were analysed in two separate samples of venous blood during first 48 hours. Neurological and systemic complications, functional status on discharge (modified Rankin scale, defining dependence as a value greater than 2), and the probable mechanism of stroke (TOAST criteria) were recorded. Results: Fifty five patients had a TACI syndrome, 56 PACI, 71 LACI, and 18 POCI. The TACI syndrome was associated with higher values of D-dimer and D-dimer/fibrinogen rate. Considering the mechanisms, median of ESR was greater in patients with atherothrombotic or lacunar stroke. The patients with cardioembolic stroke and the controls showed the smallest values and differences among subtypes were not significant. The median of fibrinogen was significantly higher in all subtypes of stroke compared with controls. The level was greater in the cardioembolic subtype, but differences among subgroups were not significant. The medians of D-dimer and D-dimer/fibrinogen rate were higher in patients with cardioembolic stroke (p<0.0001 after compare both parameters with controls and patients with atherothrombotic or lacunar stroke; p=0.002 after compare D-dimer, and p=0.004 after compare the rate with patients with undetermined stroke). The median of CRP was significantly higher in all groups of stroke versus controls. After compare subtypes of stroke, the median of CRP was significantly higher in patients with atherothrombotic stroke versus those with lacunar (p=0.010), or undetermined (p=0.003) stroke. There were defined three biochemical profiles: (1) inflammatory; (2) prothrombotic; (3) mixed. The inflammatory one was observed in patients with atherothrombotic stroke and it was basically defined by the increase of CRP, and, secondarily, by increase of fibrinogen and ESR values. The prothrombotic profile was observed in patients with cardioembolic stroke and it was mainly characterised by the increase of D-dimer and D-dimer/fibrinogen rate, and, secondarily, by the augmentation of fibrinogen and by the lack of increase of CRP and ESR. The mixed profile was observed in patients diagnosis of lacunar or undetermined stroke and showed intermediate characteristics. On discharge, 81 patients (40.5%) were independent. The variables independently related were age (OR 1.050, CI 95% 1.010-1.092, p=0.014) and NIHSS (OR 1.576, CI 95% 1.332-1.865, p<0.0001). Seventy seven patients (30.8%) experimented complications during in-hospital period. The variables independently related to infectious complications were age (OR 1.084, CI 95% 1.033-1.138, p=0.001) and NIHSS (OR 1.343, CI 95% 1.205-1.495, p<0.0001). The variables related to progression of stroke were the antecedent of intermittent claudication (OR 12.604, CI 95% 2.297-69.144, p=0.004) and NIHSS (OR 1.156, CI 95% 1.049-1.273, p=0.003). Seven patients (3.5%) died. Mortality was associated with NIHSS (OR 1.197, CI 95% 1.041-1.376, p=0.012). Discussion: The results showed the presence of analytical alterations during the first 72 hours of acute phase of ischemic stroke. Compared with controls, patients had greater levels of fibrinogen, D-dimer, D-dimer/fibrinogen rate, ESR, CRP, leukocytes, alpha-1 and alpha-2 globulins, and lesser of albumin. As patients with severe systemic complications were excluded from study, these differences may be basically related to stroke, representing the acute phase response and the processes associated with the aetiologies of the ischemic episode. Excepting D-dimer, the lack of correlation between main laboratory variables and TACI syndromes suggests that found difference are partially related to causes of stroke. The patients with cardioembolic stroke showed a thrombogenic profile suggestive of a continuous activation of coagulation-fibrinolysis system. In these patients, raised values of D-dimer/fibrinogen rate support this mechanism. The patients with atherothrombotic stroke showed an inflammatory profile which suggested the participation of systemic inflammatory processes and endothelial dysfunction in the development of atheromatosis. The patients with lacunar stroke showed a mixed profile with predominance of inflammation. The patients with undetermined stroke showed a mixed profile with predominance of thrombogenesis, suggesting that in someone of these patients the mechanism of stroke is probably cardioembolic. The associations found between dependence and age and NIHSS score, between incidence of infectious complications and age, between the development of progressing stroke and NIHSS score, and between mortality and NIHSS score agree with results of previous studies and confirm the prognostic value of clinical evaluation of patients with stroke.
