Estudio de la correlación existente entre el efecto supresor contralateral y la fatiga auditiva mediante otoemisiones acústicas transitorias.

Abstract

Introducción: La identificación de personas susceptibles a la acción del ruido es uno de los objetivos de la medicina preventiva laboral, la realización de audiometrías a los trabajadores expuestos a ruido es una prueba complementaria para la valoración de la fatiga auditiva, el trauma sonoro y la hipoacusia producida por ruido, pero no detecta las alteraciones funcionales de las células ciliadas externas inducidas por ruido en etapas iniciales y aún reversibles, sin embargo las otoemisiones acústicas pueden detectar cambios en etapas primarias y por tanto reversibles.<br/><br/>Material y métodos: Analizamos la influencia que ejerce la estimulación acústica homo y contralateral valorando el efecto supresor contralateral y la fatiga auditiva en el registro de las otoemisiones acústicas transitorias (TEOAE) en una muestra de 67 oídos humanos. Realizamos el registro en tres condiciones: Sin estimulo, con estimulación acústica contralateral y tras estimulación acústica homolateral. Las personas que presentan un efecto supresor contralateral marcado no deberían presentar fatiga auditiva (TTS) y viceversa, por lo que estos individuos tendrían una protección frente a la exposición al ruido.<br/><br/>Resultados: La estimulación acústica homo o contralateral es capaz de provocar modificaciones de la actividad coclear objetivable a través de reducciones de las TEOAE. En el 80,6 % de los casos en que se producen reducciones de la amplitud >0,5 dB solo aparecen en uno de los dos registros obtenidos bajo la influencia de la estimulación acústica. Existe una correlación negativa estadísticamente significativa entre el efecto supresor contralateral y la fatiga auditiva. La edad guarda correlación con las amplitudes de las tres pruebas, pero no con el efecto supresor contralateral y la fatiga auditiva. <br/><br/>Discusión: La amplitud de las TEOAE disminuye cuando se emplea una estimulación acústica ipsi y contralateral debido al papel modulador del sistema eferente medial sobre las celulas ciliadas externas y en consecuencia sobre las TEOAE. El efecto supresor contralateral influye de forma directa sobre la menor susceptibilidad a la fatiga auditiva y viceversa, al desencadenar un importante efecto supresor el oído se encuentra menos susceptible a la fatiga auditiva objetivable a través de un menor cambio transitorio del umbral auditivo o por una reducción en la amplitud de las TEOAE. Podemos considerar a las TEOAE como una prueba objetiva, sensible y rápida para valorar la susceptibilidad individual a padecer una lesión por exposición al ruido al ser capaz de detectar cambios cocleares subclínicos.


Introduction: The identification of capable persons to the action of the noise is one of the aims of the preventive labour medicine, the accomplishment of the audiometry to the workers exposed to noise is a complementary test for the valuation of the auditory fatigue, the sonorous trauma and the hipoacusia produced by noise, but it does not detect the functional alterations of the cells hair induced by noise in initial and still reversible stages, on the contrary the otoacoustic emisions can detect changes in primary stages and therefore reversible.<br/><br/>Material and methods: We analyze the influence that the ipsilateral and contralateral otoacoustic stimulation exercises, valuating the suppressing contralateral effect and the auditory fatigue for the record of the transient otoacoustic emissions for a sample of 67 human ears. We made the record in three conditions: Without stimulus, with otoacoustic contralateral lateral stimulation and after otoacoustic ipsilaterallateral stimulation. The persons who present a high suppressing contralateral lateral effect, should not present auditory fatigue and viceversa, thus individuals would have a protection from the exhibition to the noise.<br/><br/>Results: The ipsilateral or contralateral otoacoustic stimulation are capable of causing modifications of the activity coclear objetivable through reductions of he TEOAE. In 80,6 % of the cases in which reductions of amplitude > 0,5 dB take place, they only appear in one of the two records obtained under the influence of the otoacoustic stimulation. There is a negative statistically significant correlation between the suppressing contralateral effect and the auditory fatigue. <br/><br/>Discussion: The amplitud of the TEOAE diminishes when an otoacoustic stimulation uses ipsilateral and contralateral, due to the modulating role of the medial efferent system on the cells hair. The suppressing contralateral effect influences directly on the minor susceptibility to the auditory fatigue and viceversa, when an important suppressing effect unleashes the ear is less capable to the auditory fatigue through a minor transient change of the auditory threshold or for a reduction in the extent of the TEOAE.
