Abstract

La infiltración del sistema nervioso central (SNC) en pacientes con hemopatías malignas conlleva un mal pronóstico, además de un deterioro en su calidad de vida. La presente Tesis Doctoral (“Avances en el diagnóstico y tratamiento, y significado pronóstico de la infiltración neuromeníngea en leucemias agudas y linfomas agresivos”) incluye artículos acerca de diferentes aspectos del diagnóstico, prevalencia y factores pronósticos de la meningosis leucémica y linfomatosa. En el artículo “Central nervous system relapse in adult patients with acute lymphoblastic leukemia: frequency and prognosis in 467 patients without cranial irradiation for prophylaxis (JM Sancho et al, Cancer 2006;106:2540-2546)”, se realiza una revisión de 467 pacientes adultos con leucemia aguda linfoblástica (LAL) tratados con protocolos del grupo PETHEMA entre los años 1989 y 2003. En dichos protocolos la profilaxis de la infiltración meníngea se basó en quimioterapia sistémica (metotrexato y citarabina a dosis elevadas) junto a tratamiento intratecal triple (TIT) con metotrexato, citarabina e hidrocortisona. Se excluyó la irradiación craneal en un intento de eliminar la toxicidad asociada a la radioterapia. El resultado principal fue una frecuencia de recaída meníngea similar (5,8%) a las de los protocolos que incluyen radioterapia. El único factor predictivo de recaída neuromeníngea fue un valor de LDH sérica > 1000 U/L al diagnóstico. Por último, se constató el mal pronóstico de los pacientes con recaída neuromeníngea, idéntico al de la recaída medular. El segundo trabajo analiza la eficacia y tolerabilidad de la formulación de citarabina liposómica de liberación retardada intratecal, administrada como uso compasivo, en la meningosis leucémica en 10 pacientes con leucemia aguda mieloblástica (LAM) de España y Reino Unido (“Results of compassionate therapy with intrathecal depot liposomal cytarabine in acute myeloid leukemia meningeosis [JM Sancho et al, International Journal of Hematology 2007;86:33-36”). Nueve de los 10 pacientes lograron la eliminación completa de blastos en líquido cefalorraquídeo (LCR), con escasos efectos secundarios. Estos resultados justifican el desarrollo de ensayos clínicos para establecer el papel de esta formulación de citarabina en la profilaxis y tratamiento de la meningosis leucémica. La citología convencional (CC) en LCR ha sido el método estándar para el diagnóstico de la infiltración del SNC, aunque su sensibilidad es limitada. Estudios recientes han demostrado que la citofluorometría (CFM) multiparamétrica mejora dicha sensibilidad, pero se desconoce el significado clínico de la presencia de enfermedad oculta en LCR (CFM+ y CC-). En el estudio prospectivo “Clinical significance of occult cerebrospinal fluid involvement assessed by flow cytometry in non-hodgkin’s lymphoma patients at high risk of central nervous system disease (JM Sancho et al, European Journal of Haematology 2010; 85:321-328)” se demuestra el valor pronóstico desfavorable de la presencia de enfermedad oculta en LCR en pacientes con linfoma agresivo, con el hallazgo de una mayor tasa de recaídas en SNC. Este resultado pone de manifiesto la conveniencia de incorporar la CFM de muestras de LCR al diagnóstico, de cara a implementar mejores y más eficaces estrategias de profilaxis de la recaída en SNC. Finalmente, dos estudios prospectivos multicéntricos (“Practice of central nervous system prophylaxis and treatment in acute leukemias in Spain. Prospective registry study [JM Sancho et al, Medicina Clínica (Barc) 2008;131:401-405]” y “Prospective study on the practice of central nervous system prophylaxis and treatment in non-Hodgkin’s lymphoma in Spain [JM Sancho et al, Medicina Clínica (Barc) 2008;131:441-446]”) describen las estrategias de profilaxis y tratamiento de la infiltración meníngea en leucemias agudas y linfomas en España. Como hallazgos principales destacan la baja utilización de la radioterapia, la utilización de la TIT como pauta intratecal más frecuente y la introducción de la formulación liposómica de citarabina, en la profilaxis y, sobre todo, en el tratamiento de la infiltración neuromeníngea.


Central nervous system (CNS) involvement in patients with hematological malignancies has a poor prognosis, and it has been associated with a decrease in the quality of life. This Doctoral Thesis entitled “ADVANCES IN THE DIAGNOSIS AND TREATMENT, AND PROGNOSTIC SIGNIFICANCE OF MENINGEAL INVOLVEMENT IN ACUTE LEUKEMIAS AND AGGRESSIVE LYMPHOMAS” include articles about diagnosis, prevalence and prognostic factors of leukemic and lymphomatous meningeosis. In the first article entitled “Central nervous system relapse in adult patients with acute lymphoblastic leukemia: frequency and prognosis in 467 patients without cranial irradiation for prophylaxis (JM Sancho et al, Cancer 2006;106:2540-2546)”, 467 adult patients with acute lymphoblastic leukemia (ALL) treated with PETHEMA protocols between 1989 and 2003 were included. CNS phophylaxis consisted of a combination of high-dose systemic chemotherapy (methotrexate plus cytarabine) and triple intrathecal therapy (TIT) with methotrexate, cytarabine and hydrocortisone. Cranial radiotherapy was omitted in order to avoid toxicity. The main result of this study was a frequency of CNS relapse (5.8%) similar to that of protocols including radiotherapy. A lactate dehydrogenase (LDH) value > 1000 U/L was the only factor found to be associated with CNS recurrence. The prognosis for patients who develop CNS recurrence was poor, identical to that for patients who develop bone marrow recurrence. The 2nd study analizes the effcacy and tolerability of intrathecal (IT) depot liposomal cytarabine, administered as compassionate use, in the treatment of leukemic meningeosis in 10 patients diagnosed with acute myeloid leukemia (AML) from Spain and United Kingdom (“Results of compassionate therapy with intrathecal depot liposomal cytarabine in acute myeloid leukemia meningeosis [JM Sancho et al, International Journal of Hematology 2007;86:33-36”). In this study, 9 out of 10 patients achieved complete clearance of blast cell in cerebrospinal fluid (CSF), with mild side effects. These results justify the development of clinical trials in order to establish the role of this new formulation of cytarabine in the prophylaxis and treatment of leukemic meningeosis. Diagnosis of CNS involvement is based on the cytologic analysis (CC) in CSF, but its sensitivity is low. Recent studies have demonstrated that flow cytometry (FCM) analysis of CSF is more sensitive than CC, but the clinical significance of occult CNS disease (positive FCM with negative CC) remains unknown. In the study “Clinical significance of occult cerebrospinal fluid involvement assessed by flow cytometry in non-hodgkin’s lymphoma patients at high risk of central nervous system disease (JM Sancho et al, European Journal of Haematology 2010; 85:321-328)” the presence of occult CNS involvement at diagnosis in patients with aggressive B-cell lymphoma was associated with a higher probability of CNS relapse. These results support the incorporation of FCM study of CSF into the evaluation of the risk of CNS relapse in aggressive B lymphomas. Finally, two multicentric prospective studies (“Practice of central nervous system prophylaxis and treatment in acute leukemias in Spain. Prospective registry study [JM Sancho et al, Medicina Clínica (Barc) 2008;131:401-405]” and “Prospective study on the practice of central nervous system prophylaxis and treatment in non-Hodgkin’s lymphoma in Spain [JM Sancho et al, Medicina Clínica (Barc) 2008;131:441-446]”) describe the strategies of prophylaxis and therapy of CNS involvement in acute leukemias and lymphomas in Spain. These studies showed that TIT was the most frequent schedule for CNS prophylaxis and therap. In addition, the lack of use of cranial or craniospinal irradiation and the increasing use of new drugs (i.e.: liposomal depot cytarabine) are of note.
