Abstract

Introducción: La osteoradionecrosis mandibular (ORN) sigue siendo una de las complicaciones más problemáticas y severas que ocurre tras la radioterapia para el tratamiento del cáncer de cabeza y cuello, y que se caracteriza por la exposición ósea de un área previamente irradiada sin evidencia de curación. Su manejo clínico es complejo y requiere en muchos casos un tratamiento rápido y agresivo. Recientemente, una nueva teoría sobre su fisiopatología ha proporcionado un nuevo enfoque terapéutico basado en las propiedades antioxidantes y antifibróticas de los fármacos Pentoxifilina y Tocoferol (PVe). Hipótesis y Objetivos: Determinar la eficacia del protocolo PVe en el tratamiento de la ORN evaluando la regresión del área de exposición ósea intraoral calculada en mm2. Métodos: Ensayo clínico prospectivo randomizado en 25 pacientes diagnosticados de ORN. El grupo experimental recibió diariamente pentoxifilina 800mg y Tocoferol 1000UI durante un mínimo de 6 meses y un máximo de 18, mientras que el grupo control realizó el tratamiento estándar del servicio. El análisis clínico y radiológico se realizó antes del tratamiento y tras 1, 3, 6, 12 y 18 meses de su inicio (M1, M3, M6, M12, M18). Resultados: Una reducción estadísticamente significativa de la exposición ósea fue observada en el 85% (11/13) de los pacientes del grupo experimental tras 3 meses de tratamiento alcanzando la curación completa el 69% (9/13) a los 18 meses (M3p=0,049, M6p=0,029, M12p=0,029 y M18p=0,017) con un tiempo medio de cicatrización de 13 meses (rango: 3-18). Además se objetivó un aumento significativo del grado de apertura oral de +0,69mm a partir del tercer mes (M3p=0,049). El análisis radiológico de la microestructura trabecular mostró una mejoría de la calidad ósea no significativa en el grupo experimental. Ningún paciente presentó efectos adversos o secundarios. Conclusión: El protocolo PVe ha mostrado ser una terapia efectiva para la regresión clínica y radiológica de la ORN en pacientes con una longitud de exposición ósea intraoral menor a 3cm.


Introduction: Mandibular osteoradionecrosis (ORN) remains one of the most problematic and severe complications that occur after radiotherapy for the treatment of head and neck cancer and is characterized by bone exposure in a previously irradiated area with no evidence of cure. Its clinical treatment is complex and requires in many cases a fast and aggressive treatment. Recently, a new theory on pathophysiology has provided a new therapeutic approach based on the antioxidant and antifibritic properties of the drugs Pentoxifylline and Tocopherol (PVe). Hypothesis and Objetives: The aim of this study was to determine the efficacy of these drugs in the treatment of this pathology by evaluating the regression of the area of intraoral bone exposure calculated in mm2. Methods: Prospective study performed on 25 patients diagnosed with ORN. The experimental group received daily pentoxifylline 800mg and Tocoferol 1000UI for a minimum of 6 months and a maximum of 18, while the control group realized the standard treatment. The clinical and radiological analysis was performed before the treatment and after 1, 3, 6, 12 and 18 months of its onset (M1, M3, M6, M12, M18). Results: A statistically significant reduction of mucosal ulceration was observed in 85% (11/13) patients in the experimental group after 3 months of treatment reaching complete healing 69% (9/13) at 18 months (M3p =0.049, M6p=0.029, M12p=0.029 and M18p=0.017) with a mean healing time of 13 months (range: 3-18). In addition, a significant increase of +0,69mm in mouth opening was observed after 3 months (M3p = 0.049). The radiological analysis of trabecular microstructure showed an improvement, but not significant, of bone quality in the experimental group. No patient presented adverse or secondary effects. Conclusion: The PVe protocol achieved clinical and radiological regression of ORN in patients with a bone exposure length less than 3 cm.
