Abstract

Esta investigación se enmarca dentro del proceso de realización de los estudios de doctorado de D. Juan Vicente López Díaz, en colaboración con el equipo del Servei de Medicina Física i Rehabilitació de l'Hospital de l'Esperança (Parc de Salut Mar, Barcelona). Este estudio se centra en analizar la eficacia de la fisioterapia manual oscilatoria, según el concepto denominado POLD, en las lumbalgias crónicas inespecíficas mecánico degenerativas (LCIMD) y compararlo con el tratamiento habitual que se realiza en el Servicio de Medicina Física y Rehabilitación del Hospital de la Esperança, que sigue los protocolos que la evidencia actual marca como más eficaces. Estudio analítico, del tipo ensayo clínico controlado, randomizado con ciego simple. La muestra de 50 pacientes se divide en dos grupos homogéneos, para recibir el tratamiento habitual (Control) o la intervención con la técnica POLD (POLD). Analizamos las variables de la intensidad del dolor mediante la escala de dolor analógico y visual EVA, la evolución de la discapacidad mediante el test de Oswestry y el estado de salud mediante el SF-12. En la evolución del dolor, los pacientes tratados con POLD presentaron cambios significativos en el dolor lumbar (EVA) en función de las sesiones, en todos los tiempos y se mantuvo esta mejoría, después de acabar el tratamiento, en los controles a 3 y 6 meses, con una significación estadística en todos los tiempos (P<0,001). Respecto de la variable dolor, el grupo POLD presenta mejores resultados que el grupo control en todos los tiempos del estudio, durante el tratamiento y hasta el final del mismo con una significación estadística de P<0,001, en el control a 3 meses con una significación de (P=0,001) y a los 6 meses después de terminar el tratamiento con una significación (P<0,05). En los efectos sobre el estado de salud física (SF12), el grupo POLD presenta una mejoría significativa en todos los tiempos durante el tratamiento (P<0,001), también en el control a 3 meses (P<0,001) y en control a 6 meses (P<0,05) tras finalizar el tratamiento. En la comparación entre los dos grupos, el grupo POLD manifiesta durante el tratamiento una mayor eficacia, estadísticamente significativa (P=0,001), a tres meses se mantiene la significación P<0,05, pero en el control a 6 meses la significación estadística de la diferencia entre ambos grupos desaparece (P=0,126), aunque aun así se mantiene por encima del grupo de control en sus valores absolutos de salud física. En los efectos sobre el estado de salud mental (SF12), el tratamiento POLD no presenta una eficacia estadísticamente significativa desde el inicio hasta el final del estudio, pero aun así, elevó los valores por encima de la media, para la población de Catalunya. En la variable de discapacidad (Oswestry), en el grupo POLD el tratamiento es eficaz en la reducción de la discapacidad, tanto en la primera semana de tratamiento (P<0,05), como al terminar el tratamiento (P<0,001) y a 3 y 6 meses de seguimiento (P<0,05). En la comparación entre los dos grupos el tratamiento POLD es significativamente más eficaz que el control en todos los tiempos del estudio, a la semana con 5 sesiones de tratamiento (P<0,05), a las dos semanas y 10 sesiones de tratamiento y finalizado el mismo (P<0,001), y durante el seguimiento a 3 y 6 meses (P< 0,05). En nuestro estudio, la aplicación del método POLD de fisioterapia para el tratamiento del dolor lumbar crónico mecánico degenerativo inespecífico se manifiesta eficaz en la disminución de la intensidad del dolor, la discapacidad y en la mejoría del estado de salud física, y más eficiente que el tratamiento habitual aconsejado por la evidencia actual para esta misma patología.

This study focuses on analysing the effectiveness of oscillatory manual therapy based on the POLD concept in non-specific mechanical degenerative chronic low back pain (NMDCLBP) and compares it with the usual treatment performed in the Department of Physical Medicine and Rehabilitation (“Esperança” Hospital), which follows protocols that have been proven as the most effective for this pathology to date. Analytic single-blind randomized controlled clinical trial study. A sample size of 50 patients was divided into two homogeneous groups that were applied either the usual treatment (Control) or POLD technique intervention (POLD). We analysed pain intensity by Visual Analogy Scale (VAS), health status using the SF12 test, and disability progression by Oswestry test. Regarding the pain variable, patients treated with POLD showed statistically significant changes in low back pain (VAS) throughout the treatment sessions and also at all time points compared in the study (P<0.001). The observed significant improvement was maintained after 3 and 6 months of completion of treatment (P<0.001). Importantly, the POLD group presents better results than the Control group throughout the treatment sessions (at all time points of the study and at completion of the study) with statistical significance (P<0.001), as well as at 3 months (P=0.001) and 6 months after the treatment ended (P<0.05). On physical health (SF12), the POLD group presented a statistically significant improvement in all time points during treatment (P<0.001), as well as at 3 months (P<0.001) and 6 months (P<0.05) after treatment completion. In comparison with the Control group, the POLD group was shown more effective during treatment (at the 5th and 10th session) (P=0.001) and this significance was maintained at 3 months after treatment (P<0.05). Despite at 6 months the statistical significance between the two groups disappeared (P=0.126), SF12 values on physical health for the POLD group were still higher than those of the Control group. On mental health (SF12), the POLD treatment did not present statistically significant efficacy at any time point of the study. Nevertheless, SF12 values on mental health for the POLD group were higher than the standards registered for the Catalonian population. In the assessment of disability (Oswestry), POLD treatment was effective in reducing disability at completion of the first week of treatment (5 sessions) (P<0.05), at the end of treatment (10 sessions) (P<0.001) and at the 3 and 6 months follow-up (P<0.05). In the comparison between the two groups, POLD treatment was significantly more effective than the Control group at all time points of the study: after one week of treatment with P<0.05, after two weeks of treatment with P<0.001, and at the 3 and 6 months follow-up (P<0.05). In this study, the application of POLD method of physical therapy for the treatment of non-specific mechanical degenerative chronic low back pain was effective in decreasing pain intensity and disability and in improving physical health. Moreover, it was more efficient than the usual treatment advised by the current evidence for the same pathology.
