Abstract

El projecte principal d'aquesta tesi és la valoració de la resposta immune induïda per la vacuna RUTI®en el context d'un estudi de Fase 1. La hipòtesi fromulada fou que la RUTI®, per ser una vacuna generada a partir dels fragments de M.tuberculosis cultivats en estrès, pot generar una resposta poliantigènica segura en pacients no infectats. Per això, l'objectiu fou dissenyar i dur a terme l'avaluació de la immunogenicitat de la vacuna RUTI®per primera vegada en humans. L'estudi va implicar dues fases: la preparació i l'execució, d'un any i mig de duració, que foren seguides d'un altre any per a analitzar les dades obtingudes i la conseqüent redacció dels documents pertinents. La preparació va incloure el treball conceptual de disseny de l'estudi; l'elecció i l'establiment de tècniques que serien utilitzades en l'execució; l'organització logistíca de l'execució i el redactat dels procediments específics referents a les tècniques. Durant la planificació varen sorgir dubtes conceptuals que es varen haver de resoldre i varen generar dos estudis menors, que també s'han inclòs: 1) l' avaluació de la producció d'autoanticossos en sèrum de ratolins vacunats amb RUTI® i 2) la detecció d' ITBL durant l'screening: avaluació de la influència del TST sobre les respostes immunològiques mesurades per TIGRA. El primer estudi va demostrar que la vacunació amb RUTI® no indueix ANA detectables en sèrum però que la infecció per M.tuberculosis sí, i de forma independent del tractament administrat. La conclusió de l'estudi número 2 fou que en realitzar el TST quinze dies abans de començar el seguiment dels voluntaris, no es podia excloure en absolut detectar mitjançant TIGRA unes respostes immunològiques alterades fictíciament, fins i tot tractant-se d'individus sans. Els resultats obtinguts foren suficientment alarmants com per a decidir no incloure el TST a l'screening de l'assaig de Fase 1. El veritable cos del projecte, l'avaluació de la immunogenicitat de RUTI®en humans, es descriu al text com a estudi número 3. Dut a terme en el context d'un assaig clínic de Fase 1, pretenia avaluar la immunogenicitat de 4 dosis diferents de la vacuna en individus sans. Testades seqüencialment per tal de detectar qualsevol efecte secundari que pugués posar en perill la seguretat dels voluntaris, cadascuna de les dosis fou assajada en 4 voluntaris sans dins d'un grup en què també participaven 2 voluntaris que foren vacunats amb placebo. La vacuna es va administrar dues vegades separades per 28 dies, i el seguiment dels participants es va allargar a 6 mesos cadascun. Donat que abans de començar l'assaig de Fase 1, es desconeixien els possibles efectes de la vacuna RUTI®en humans i que aquesta havia estat dissenyada per al tractament de la ITBL, es desconeixia si en població sana la RUTI®tindria la força suficient per a generar una resposta específica en contra de M.tuberculosis que fos mesurable. Per això es va incloure un rang tan ampli de tècniques amb dues finalitats: 1) per a avaluar l'impacte de la vacunació sobre les respostes immunològiques, per tal de detectar qualsevol resposta deguda a la RUTI®, i 2) determinar quins assajos serien útils en els futurs estudis del desenvolupament clínic. Els resultats de les tècniques que avaluaven la resposta cel·lular van demostrar una tendència dosi-resposta per la majoria d'assajos, assenyalant la dosi de 25µg de FCMtb com la més poliantigènica. L'avaluació de la resposta humoral va demostrar una pauta d'increment d'IgM enfront els antígens de M.tuberculosis amb uns nivells d'IgG baixos similar a la que existeix en individus contactes de malalts tuberculosos. En quant a relació entre els resultats de l'avaluació de seguretat i la d'immunogenicitat, si bé els EA locals semblaren mantenir certa correlació amb l'assaig de memòria únicament, una resposta immunològica específica deguda a la vacunació amb RUTI®no va implicar una més alta incidència d'EA. Les conclusions del treball serien que: 1) la vacuna RUTI®és immunogènica en individus sans, 2) les respostes immunològiques de tipus cel·lulars aconseguides en general seguiren una tendència dosi-resposta, obtenint la resposta més poliantigènica amb la dosi de 25µg de FCMtb; i 3) com a experiència de l'estudi, en la realització en assajos futurs caldria incloure en el desenvolupament clínic de la vacuna: a) l'avaluació de la immunitat cel·lular mitjançant ELISPOT enfront PPD i els antígens de M.tuberculosis ESAT-6, 85B, i 16 i 38kDa; b) l'assaig de memòria; i c) l'avaluació de la resposta humoral enfront els mateixos antígens.


The original project of this thesis was the evaluation of the immune response induced by the antituberculous vaccine named RUTI¨ in the context of a Phase 1 Clinical Trial. The hypothesis of the study was that RUTI¨, for being generated from fragments of M.tuberculosis grown in stressed cultures, could trigger a safe and poliantigenic response in healthy volunteers. To prove the hypothesis, the objective was to design and perform the evaluation of RUTI¨'s immunogenicity in humans, for the first time. The study implied two phases: the preparation, which lasted 1 and a half years, and another year and a half for the execution and analyisis of all data, with the subsequent reporting. The preparation included the design of the study, the selection and set-up of all the techniques, the organisation, and the generation of the specific procedures for all the assays. During the planning, some conceptual doubts arisen, thus 2 minor studies were designed and performed to solve them, both of them being included in the thesis: 1) evaluation of the autoantibodies' production in the serum of RUTI¨ vaccinated mice; and 2) detection of ITBL by TIGRA during the screening: evaluation of TST influence on the immune responses measured by TIGRA. The first study demonstrated the vaccination with RUTI®does not generate detectable ANA in serum, but M.tuberculosis itself does regardless the treatment. The conclusion of study number 2 was that to perform the TST fifteen days before starting the follow-up of the volunteers could influence the results of the immune responses measured by TIGRA, even in healthy people. The results obliged us to exclude the TST from the screening of the volunteers to be included in the Phase 1 CT. The original project -the evaluation of the immunogenicity of RUTI®when administered to humans-has been included in the thesis as study 3. Performed in the context of a Phase 1 CT, it aimed to evaluate the immunogenicity of 4 different doses of the vaccine in healthy volunteers. Sequencially tested (in order to detect any adverse effect who could impair the safety), every dose was administered to 4 volunteers of a group of 6, where the other 2 received placebo. The vaccine was administered twice (28 days apart), and the follow-up of each participant lasted 6 months. As no effect of RUTI®in humans was previously known and the vaccine was designed to treat the LTBI, the doubt was if the vaccine could be able to generate a sufficient specific response against M.tuberculosis to be detected. To determine this, a wide range of assays were included, with 2 aims: 1) to evaluate the impact of the vaccination on the immune responses and 2) to determine which assays could be useful in future clinical trials to be conducted during the development plan of the vaccine. The results of the assays intending to evaluate the cellular immune response demonstrated a dose-response tendency for most of them, signaling the dose of 25µg of FCMtb as the most poliantigenic one. The evaluation of the humoral response demonstrated a pattern with an increase of IgM antibodies against M.tuberculosis antigens together with IgG antibodies levels, similar to the one found in contact people of M.tuberculosis patients. Regarding the relationship between the results obtained in both the evaluation of the safety and of the immunogenicity, even if the local AE seemed to have some correlation with the memory assay, a specific immune response due to the vaccination did not imply a higher incidence of AE. The conclusion of the study are: 1) the vaccine RUTI®is immunogenic in healthy individuals; 2) the immune responses obtained in general followed a dose-response tendency, the dose of 25µg of FCMtb being the most poliantigenic; and 3) as result of the study, we would recommend to include the following assays in future clinical trials: a)the evaluation of cellular immune response by ELISPOT against PPD and the following M.tuberculosis antigens: ESAT-6, 85B, and 16 and 38kDa; b) the memory assay; and c) the evaluation of the humoral immune response against the same M.tuberculosis antigens.
