Abstract

El microorganisme bacterià més virulent del món és Mycobacterium tuberculosis ja que és el que mata més persones per si sol que cap altra infecció. Encara que la malaltia es tracta amb quimioteràpia i existeix una vacuna preventiva existeix un reservori de 1.800.000.000 persones infectades en el món en aquest moment; això representa gairebé un terç de la població mundial. <br/>L'home és un hostatger resistent a la infecció per M. tuberculosis i per això caldria trobar un model animal que es comporti de forma semblant. En aquest sentit, les soques resistents a la infecció tuberculosa podrien reproduir aquesta situació i ens permetrien estudiar el comportament del bacil en unes condicions semblants a les humanes.<br/>Un macròfag inactivat no pot destruir M. tuberculosis i permet el seu creixement i supervivència dins del fagosoma afavorit per mecanismes de depressió immunològica. En canvi, en ser activat el macròfag, es converteix paulatinament en una màquina efectiva de destrucció que comporta l'acidificació del fagosoma, condicions reductores, hidrolases i efectes potencials de ROIs i RNIs. Aquests mecanismes potencialment destructors no semblen ser definitivament efectius davant la infecció i així parlem ocasionalment de bacils latents. Aquests bacils han de romandre durant un temps perllongat en aquestes condicions passant desapercebuts als mecanismes immunològics i suposant un dels principals problemes de la tuberculosi, la latència.<br/>Segons les hipòtesis actuals de la dinàmica de poblacions als granulomes que fonamenten el tractament de la tuberculosi, existirien poblacions semi-latents a les ferides granulomatoses que romandrien extracel·lularment i a un pH àcid (pH<5,5). Aquesta població seria una de les causants del fracàs del tractament junt amb la població latent. És per això que l'estudi de la susceptibilitat a l'àcid en un model in vitro ens pot permetre conèixer una mica més el comportament del bacil davant aquest mecanisme d'estrès. Existeixen ja altres estudis semblants amb M. smegmatis i fins i tot amb M. tuberculosis, però tots han basat la seva metodologia en shocks àcids, és a dir, adaptació ràpida a aquesta condició o com a molt han mantingut el bacil durant vàries hores. Realment, no sembla que sigui aquesta l'estratègia real que adopta el macròfag davant la infecció tuberculosa, ja que tot el procés descrit anteriorment per la caracterització del fagosoma i la posterior maduració a fagolisosoma porta un temps relativament curt, però M. tuberculosis roman més temps i cal un estudi tenint en compte aquesta variació de temps. A més, la destrucció del bacil per mecanismes clàssics del fagolisosoma no és immediata i pot no portar a una degradació total. De la mateixa forma, l'estudi en el model murí per aerosol de l'adaptació metabòlica de M. tuberculosis ens ha de servir per a corroborar els resultats obtinguts in vitro.<br/>Aquest treball intenta donar més llum sobre el bacil latent a nivell genètic i immunològic basant-se en estudis de metabolisme i resposta a l'estrès en un model in vitro i en el model murí per aerosol. L'objectiu essencial serà doncs el de cercar marcadors moleculars que permetin determinar la presència de bacils latents en teixit. Concretament, aquest estudi ha permès conèixer com varia l'expressió de més de 60 gens de M. tuberculosis a la presència d'àcid en un model in vitro que podria reproduir les condicions presents al interior del macròfag. Posteriorment, s'ha estudiat l'expressió dels gens més rellevants en ratolins infectats per aerosol aconseguint informació sobre el comportament d'aquest bacil dins del seu hostatger durant tot el procés d'infecció mitjançant la caracterització immunològica d'un model crònic per l'estudi de 24 citocines. Prèviament, s'ha caracteritzar àmpliament la patogènia d'aquest model crònic comparant la susceptibilitat a la infecció de dues soques de ratolí.


The most striking virulent organism in the world is Mycobacterium tuberculosis because it kills 2 million people every year all alone. An effective treatment and vaccine are available, but 1.800.000.000 people are still infected in the world, just a third of the world population.<br/>Tuberculosis only develops in 10% of immunocompetent people, so a resistant phenotype is shown. So, we have to look for a resistant model to resemble these conditions. On the one hand, mice resistant strains to tuberculosis are a good model for this study.<br/>An inactivated macrophage is not able to kill M. tuberculosis and allows the exponential growing and surivival inside phagosome in absence of an effective immune response. On the other hand, correctly activated macrophage acts as an effective destroy machine by acidificaction of phagosome, reduction condition, hydrolases and hazard effects of ROIs and RNIs. These mechanisms of destructions do not seem to be totally effective and that's the reason why we have to talk about latent infection. These bacilli must remain in those conditions allowing immune response not to recognise them and rising as one of the most striking problems in tuberculosis, latency.<br/>According to actual hypothesis about dynamic population in granulomes that are the basis for the treatment of tuberculosis, some semi-latent population would be present in granulomatous injuries extracellularly at pH lower than 5,5. This population would be one of the main reason for the treatment fail in conjunction with latent bacilli. That's the reason why the study of acidification susceptibility in an in vitro model may allow us to deeply know the bacilli behaviour in front of stress mechanisms. M. smegmatis has been studied in order to determine its answer to those conditions, but not M. tuberculosis. All strategies performed are based on a acid shock, this is a fast adaptation, but this situation does not resemble real conditions. I mean that adaptation to acidic conditions is related to a process of several hours (about 6). Moreover, phagosome maduration and phagolysosome formation is a slow process that requires long time. So, this time modulation must be taken into account. In the same way, the killing of M. tuberculosis may not be a fast process, this is, bacilli are able to overcome acidification for some days and so on. Eventually, the performing of this idea in the murine model is related to a metabolic adaptation of M. tuberculosis and must be useful for the confirmation of in vitro results.<br/>This study is trying to add some more ideas about the latent bacilli at genetic and immunologic level based on metabolism studies and answer to stress in an in vitro and in vivo model. The essential objective is to look for moleculars markers that may allow to define the presence of latent bacilli in tissues. In detail, this study has monitored the evolution of more than 60 genes of M. tuberculosis related to the acidic response that may resemble conditions in phagosomes from macrophages. Later, this expression has been followed up in a murine mode according to the conditions studied before. On the other hand, the immune response has been studied according to the expression of 24 cytokines related to tuberculosis. Previously, we have compared the behaviour of M. tuberculosis in two different strains of mice providing evidence of a resistant model for C57BL/6 mice.
