Abstract

Actualmente la cicatrización hipertrófica y queloidea afecta a un número considerable de pacientes post-quemados. Representa la primera causa de morbilidad y complicaciones en el paciente quemado superviviente, sin que se disponga de ningún tratamiento único definitivo ni suficientemente eficaz, generando un gran gasto sanitario. La presoterapia y la silicona tópica son los métodos clásicos y los más empleados hoy en día para la prevención y tratamiento de las cicatrices patológicas en quemados. Sin embargo, sólo tienen resultados aceptables y con evidencia científica escasa, ya que en muchos casos no evitan la aparición de secuelas cicatriciales. Por estos limitados resultados, nuevas terapias en el tratamiento de las cicatrices post quemaduras actualmente están bajo investigación. La terapia mediante ondas de choque apareció en la práctica clínica para fragmentar cálculos renales y, recientemente, ha encontrado nuevos campos de aplicación tales como enfermedades musculo-esqueléticas y úlceras. Estudios experimentales sugieren que promueve la angiogénesis y reduce la isquemia tisular. Disminuye la infiltración leucocitaria y por tanto la inflamación. Inflamación y fibrosis están íntimamente relacionadas. La terapia de ondas de choque parece que aumenta la eNOS, inhibiendo así la transición epitelial-mesenquimal, responsable de la invasión tumoral y la fibrosis. La cicatriz es fibrosis, por lo tanto, la aplicación de dicha terapia en cicatrices postquemadura, podría disminuir su progresión y por lo tanto reducir la morbilidad, y, mejorar la calidad de vida del paciente. Entre los meses de julio de 2013 y agosto de 2015, cuarenta pacientes que presentaron una quemadura intermedia profunda, y que no precisó de cirugía para su curación fueron sometidos a una terapia con ondas de choque para prevenir y mejorar las cicatrices en el servicio de Cirugía Plástica y Quemados del Hospital Universitari de la Vall d’Hebron. Se empleó el propio paciente como control. Se aplicó el tratamiento solo en la zona inferior de la cicatriz. Además, en toda la cicatriz (incluida el área tratada con dermaPACE®) se aplicó el tratamiento clínico estándar habitual, presoterapia y tratamiento tópico con silicona. La cicatriz se valoró mediante la escala de Vancouver, registros fotográficos y antropométricos, el tewameter, que realiza la medición de la hidratación de la piel. Se realizó una biopsia post-tratamiento a los seis meses para evaluar el efecto de la terapia mediante ondas de choque sobre la cicatrización en comparación con la otra mitad. Los resultados obtenidos mediante el análisis estadístico derivado de la Escala de Vancouver y el dispositivo no invasivo para la medición de la hidratación de la cicatriz, el tewameter dan una significación p<0,05. Objetivan unos resultados estadísticamente favorables en cuanto a términos de humedad relativa en la cicatriz. Corroboran las mejoras en la disminución del eritema, del grosor y de un aumento en la elasticidad de la cicatriz, que ya parecían observarse a lo largo del estudio y en las fotografías realizadas y que la mayoría de los pacientes confirmaron dicha mejoría con su opinión subjetiva. En cuanto a los resultados de la biopsia, no muestran diferencias significativas, aunque muestra que en ninguna biopsia se objetiva inflamación crónica y que el nivel de fibrosis es leve, incluso a veces muy leve, que ayudarían a reforzar la teoría inicial de que las ondas de choque podrían disminuir la síntesis exagerada de colágeno. El estudio sugiere que este tratamiento no invasivo, realizado ambulatoriamente, con reincorporación inmediata a las actividades de la vida diaria, que se puede aplicar conjuntamente con otros tratamientos, ofrece una mejoría estadísticamente significativa de los resultados, con mínima morbilidad, que de confirmarse los resultados con un estudio de mayor potencia, sería otro recurso a incluir en la práctica clínica diaria de las unidades de quemados.


Nowadays hypertrophic scarring and keloid affects a significant number of post-burned patients. It represents the main cause of morbidity and complications in surviving post-burned patients, without an effective treatment, generating large health cost. Pressure therapy and topical silicon sheets are the traditional methods and still the most used for the prevention and treatment of pathological scarring in burned patients today. However, they only have acceptable scientific evidence and poor results, because in many cases they do not prevent the appearance of scarring sequelae. According to these limited results, new therapies in the treatment of post burned scars are currently under investigation. The shockwave therapy appeared in clinical practice to break up kidney stones and has recently found new applications such as musculoskeletal diseases and ulcers. Experimental studies suggest that it promotes angiogenesis and reduces tissue ischemia. It decreases leukocyte infiltration and hence inflammation. Inflammation and fibrosis are closely related. The shock wave therapy appears to increase eNOS (, thereby inhibiting epithelial-mesenchymal transition which is responsible for tumor invasion and fibrosis. The scar is fibrosis, therefore, the application of this therapy in post-burn scars could limit their progression and thereby reduce their morbidity and improve patients’ quality of life. Between July 2013 and August 2015, forty patients who had a deep intermediate burn, and that did not require surgery to heal, underwent a therapy with shock waves to prevent and improve scarring in the service of Plastic Surgery and Burn Unit of the University Hospital of Vall Hebron. The patient was used as control himself. Treatment was applied only in the lower area of the scar. In addition, standard treatment, which includes pressotherapy and topical silicone sheets was applied in the whole scar (that included the area treated with dermaPACE®). The scar was assessed by the Vancouver scale, photographic and anthropometric records were taken with a Tewameter, that measures the skin hydration. a biopsy was performed six months after the treatment to evaluate the effect of shockwave therapy on wound healing compared to the other half that did not receive the treatment. The results obtained by statistical analysis derived from the Vancouver scale and non-invasive device for measuring hydration of the scar, the Tewameter, both gave a significance evidence of p <0.05. It objectifies a statistically favorable result in terms of relative humidity in the scar. It corroborates the improvement in reducing erythema, thickness and an increase in the elasticity of the scar, which seemed throughout the study and the photographs taken and that most patients confirmed this improvement with its subjective opinion. As for the biopsy, results show no significant difference, although any biopsy reflected signs of chronic inflammation and the level of fibrosis was mild, even very slight, these facts would help strengthen the initial theory that the shock waves therapy may decrease exaggerated collagen synthesis. The study suggests that this non-invasive treatment, performed on an outpatient basis, with immediate return to daily live activities, which can be applied simultaneously with other treatments, provides a statistically significant improvement of the results, with minimal morbidity. If these results were confirmed in wider study with more statistical power, shock wave therapy would be another treatment to be included in clinical practice in burn units.
