Evaluación de la efectividad de la psicoeducación grupal en depresión mayor (síntomas leves/ moderados) en atención primaria

Abstract

Objetivos del estudio: Evaluar la efectividad de una intervención grupal psicoeducativa para la depresión mayor (síntomas leves/ moderados) en pacientes atendidos en Atención Primaria (AP). Métodos: El estudio es un ensayo clínico controlado aleatorizado de una muestra de 231 personas mayores de 20 años con un diagnóstico de depresión mayor según el CIE-10 atendidos en 13 equipos de AP de Barcelona. De la muestra total, 85 presentaban un episodio leve (BDI .9 y 018) y 146 un episodio moderado (BDI.19 029) según el cuestionario Beck Depression Inventory (BDI). El grupo intervención (GI) (n =119) recibió un programa grupal psicoeducativo (12 sesiones, 90 minutos cada sesión dirigidas por dos enfermeras) y el grupo control (GC) (n = 112) recibió la atención habitual. Los pacientes fueron evaluados al inicio, post-intervención (3 meses) y a los 6 y 9 meses de seguimiento. Las principales medidas de resultado fueron el BDI, remisión de la sintomatología depresiva basada en el BDI y EuroQol-5D-3L (EQ-5D). También se realizó un sub-análisis con aquellos pacientes que nunca habían recibido tratamiento farmacológico antidepresivo antes de formar parte del estudio. En este caso la muestra estaba formada por 106 pacientes (50 del GI y 56 del GC). Para valorar el impacto de esta intervención en la calidad de vida relacionada con la salud (CVRS), se realizó un sub-análisis con aquellos pacientes que habían completado correctamente el EQ-5D en el momento inicial, post-intervención y durante el seguimiento a los 6 y 9 meses. En este caso la muestra estaba formada por 224 pacientes (115 del GI y 109 del GC). Resultados principales: Los pacientes con síntomas depresivos leves del grupo intervención tuvieron una mayor remisión de los síntomas comparado con el grupo control, 56.80% vs. 31.30% en el momento post-intervención (p= 0.009) y 64.90% vs. 37.50% en el seguimiento a los 9 meses (p=0.006). En esta misma muestra de pacientes, se obtuvieron diferencias estadísticamente significativas entre grupos en el BDI después del tratamiento (p=0.016; d'= .51) y a los 9 meses de seguimiento (p=0.048; d'=.44). En la muestra general y en el subgrupo con síntomas moderado, los análisis sólo mostraron diferencias significativas entre grupos en el BDI post-intervención, p= 0.02 (d'=. 29) y p= 0.010 (d'=. 47), respectivamente. En la sub-muestra de pacientes sin tratamiento farmacológico al iniciar el estudio, se observaron diferencias significativas en la remisión de los síntomas post- intervención y a los 6 meses de seguimiento (p=0.001 y 0.012 respectivamente), pero a los 9 meses sólo se observaron diferencias con tendencia a la significación estadística (p= 0.064). Los análisis también mostraron diferencias significativas en el BDI post-intervención (p= 0.008; d '=. 55), pero no a los 6 y 9 meses. En relación a la CVRS, los resultados mostraron diferencias significativas entre grupos en el EQ-5D Índice a los 3 meses de seguimiento en la muestra general (p=0.022; d’=0.41) y para el subgrupo con síntomas moderados (p=0.045; d’=0.47). También se encontraron diferencias significativas en el EQ-5D VAS a los 3 meses ( muestra general p=0.008 y d’=0.48; pacientes moderados p=0.016 y d’=0.57). No se encontraron diferencias entre grupos a los 6 y 9 meses. Conclusiones: La intervención grupal psicoeducativa es un tratamiento eficaz a corto y a largo plazo para los pacientes con síntomas depresivos leves, y sólo a corto plazo en los pacientes con síntomas moderados. También muestra ser efectiva a corto plazo en aquellos pacientes no tratados con medicación antidepresiva antes de recibir la intervención psicoeducativa. La intervención mejora la CVRS a corto plazo y esta mejora se mantiene estable durante el seguimiento a los 9 meses, especialmente en el grupo de pacientes con síntomas moderados.


Study objectives: To evaluate the effectiveness of psychoeducational group therapy for major depression (mild/moderate symptoms) in Primary Care patients. Methods: The study is a controlled, randomised clinical trial on a sample of 231 people over 20 years of age diagnosed with major depression according to the ICD-10 seen in 13 Primary Health Care in Barcelona. Out of the total sample, 85 presented a mild episode (BDI +9 and -18) and 146 a moderate episode (BDI +19 and -29) according to the Beck Depression Inventory (BDI). The treatment group (TG) (n = 119) received a psychoeducational group programme (12 sessions of 90 minutes each, run by two nurses), while the control group (CG) (n = 112) received the usual treatment. Patients were assessed at the start, after therapy (3 months), and at 6 and 9 months follow-up. The principal endpoints were the BDI, remission of depressive symptoms based on the BDI and the EuroQoL-5D-3L (EQ-5D). A sub-analysis was also carried out on patients who had never received pharmacological antidepressant treatment before taking part in the study. In this case, the sample was made up of 106 patients (50 in the TG and 56 in the CG). To assess the impact of this treatment on the health-related quality of life (HRQoL), a subanalysis was carried out on patients who had correctly completed the EQ-5D at baseline, after therapy and at 6 and 9 months follow-up. In this case, the sample comprised 224 patients (115 in the TG and 109 in the CG). Main results: The patients with mild depressive symptoms in the treatment group showed a greater remission of the symptoms compared with the control group: 56.80% vs. 31.30% after therapy (p = 0.009) and 64.90% vs. 37.50% at 9-month follow-up (p = 0.006). In the same sample of patients, statistically significant differences were obtained between groups in the BDI after therapy (p = 0.016; d’ = 0.51) and at 9-month follow-up (p = 0.048, d’ = 0.44). In the general sample and in the subgroup with moderate symptoms, the analyses only showed significant differences between groups in the post-therapy BDI: p = 0.02 (d’ = 0.29) and p = 0.010 (d’ = 0.47) respectively. In the subgroup of patients without pharmacological treatment on starting the study, significant differences were observed in the remission of symptoms post-therapy and at 6-month follow-up (p = 0.001 and 0.012 respectively), but the observed differences only tended toward statistical significance at 9-month follow-up (p = 0.064). The analyses also showed significant differences in the BDI after therapy (p = 0.008, d’ = 0.55), but not at 6 and 9 months. Considering HRQoL, the results showed significant differences between groups in the EQ-5D index at 3-month follow-up in the general sample (p = 0.022, d’ = 0.41) and for the subgroup with moderate symptoms (p = 0.045, d’ = 0.47). Significant differences were also found in the EQ-5D VAS at 3 months (general sample p = 0.008 and d’ = 0.48; moderate patients p = 0.016 and d’ = 0.57). No differences were found between groups at 6 and 9 months. Conclusions: Psychoeducational group therapy is an effective short- and long-term treatment for patients with mild depressive symptoms, but only in the short term for patients with moderate symptoms. It is also shown to be effective in the short term for patients who are not treated with antidepressant medication before receiving the psychoeducational therapy. The therapy improves HRQoL in the short term, and this improvement remained stable during follow-up over 9 months, especially in the group of patients with moderate symptoms.
