Abstract

La participació del pacient en els sistemes de notificació d’esdeveniments adversos (EAs) s’ha proposat com estratègia per implicar al pacient en la seva seguretat i fer-lo coresponsable de l’ assistència sanitària. Sense qüestionar la bondat del canvi de paradigma (del pacient objecte al pacient subjecte) i la utilitat a l’entorn del pacient crònic de les estratègies d’apoderament, el treball vol donar resposta a la pregunta de si els pacients estan capacitats per a actuar com a identificadors d’esdeveniments adversos, plantejant seriosos dubtes al respecte. Volen els pacients assumir aquest rol o potser hem tornat a decidir el millor per al pacient, sense el pacient.? Amb aquesta estratègia d’implicació dels pacients en la prevenció d'errors, estem simplement desplaçant la responsabilitat de la seguretat dels proveïdors i institucions als propis pacients? L’objectiu principal d’aquest treball és identificar la concordança entre els EAs identificats pels pacients a través d’un qüestionari dissenyat a l’efecte i els EAs identificats pels professionals a través d’un sistema de notificació d’EAs. Objectius secundaris al mateix són determinar la incidència d’EAs segons els pacients, determinar si la declaració d’EAs te relació amb la percepció de seguretat dels pacients, determinar si les notícies d’errors mèdics aparegudes en mitjans de comunicació tenen relació amb la percepció de seguretat dels pacients, i determinar si els pacients informats, que han participat en el procés de decisió i de cures, tenen millor percepció de seguretat. Es tracta d’un estudi observacional longitudinal, la recollida de dades es realitza entre el 14 de març de 2011 i el 29 de maig de 2012. S’han inclòs tots els pacients majors de 18 anys (en el cas de pacients amb la cognició alterada respon el cuidador principal) donats d’alta de la Unitat de Cirurgia de l’Hospital de Sabadell en el període definit per a l’estudi, independentment de la seva procedència fins a aconseguir una mostra de 335 episodis (si un pacient ingressa dues vegades en el període d’estudi s’avalua de forma independent). Els episodis en els quals els pacients han identificat algun EA ha estat del 21,1% (IC95%= [16,8 a 26]). El total d’EAs identificats pels pacients són 99, el que suposa 1,39 EA/ per pacient. La sensació de seguretat declarada pels pacients presenta una relació inversa amb significació estadística amb la declaració d’incidents (p=0,001) i EAs (p=0,02). S’observa una tendència a l’augment de declaració d’EAs (p=0,91) entre els que consideren més freqüents els incidents. No sembla haver relació (p=0,08) entre la declaració d’EAs i la probabilitat declarada de patir un error. La declaració d’EAs, sembla no tenir relació (p=0,76) amb el fet d’haver vist o escoltat fa poc alguna noticia en relació amb un incident en l’atenció sanitària (a la premsa, la televisió, la ràdio o Internet). Els pacients que es senten molt segurs expressen un nivell d’informació/satisfacció superior a la resta de pacients (amb una mitjana de 95,23 (±7,89) respecte a 86,8 (±12,15) els bastant segurs i 80,6 (±15,7) els ni segurs ni insegurs). Només en el 36% dels casos en els que els professionals han detectat un EA també en declaren els pacients, la proporció de concordança d’identificació positiva per part de pacients i professionals és del 12%. Analitzada la concordança de forma detallada, entre la declaració d’EAs concrets per part dels pacients i la declaració dels mateixos pels professionals, trobem que en tots els casos el grau d’acord és discret i inclús sense acord (k≤0,4). Concloem doncs, que es confirma la nostra hipòtesi de baixa concordança i aporta dades en contra del suggeriment present a la comunitat científica en quant a la fiabilitat dels pacients com a identificadors d’EAs, en la seva concepció clínica.


The participation of the patient in the reporting systems for adverse events (AEs) has been proposed as a strategy to imply the patient in his safety and to make him co-responsible of the healthcare. Without questioning the goodness of the paradigm shift (from patient object to patient subject) and the usefulness around the chronic patient empowerment strategies, the work wants to answer the question about the ability of the patients to act as identifiers of adverse events, raising serious doubts towards it. Do the patients want to assume this role or have we maybe decided again what’s best for the patient without him? With this strategy of involving patients in preventing errors, are we simply shifting the responsibility for safety of providers and institutions to the actual patients? The main objective of this work is to identify the agreement between the AEs identified by the patients, through a questionary designed for this purpose, and the AEs identified by professionals, through an AEs notification system. Some secondary objectives are to determine the incidence of AEs according to the patients, to define if the statement of AEs is related to the safety perception of the patients, to decide if the news of medical errors appeared in the media are related to the safety perception of the patients, and to determine if the informed patients, who have participated in the decision and care process, have a better safety perception. This is an observational and longitudinal study. The data collection is done between the 14th of March of 2011 and the 29th of May of 2012. We have included all patients over 18 (in the case of altered cognition patients the main carer responds) registered at the Surgery Unit of the “Hospital de Sabadell” within the defined period for the study, regardless of their origin, until achieving a sample of 355 episodes (if a patient is admitted twice during the study period it is evaluated independently). The episodes in which the patients have identified any AEs have been of 21,1% (IC 95%=[16,8 a 26]). The total AEs identified by the patients are 99, which means 1,39 EA/per patient. The safety feeling declared by the patients presents a statistically significant inverse relation with the declaration of incidents (p=0,001) and AEs (p=0,02). There is a tendency to increase the statement of AEs (p=0,91) amongst those who consider incidents to be most frequent. There doesn’t seem to be a relation (p=0,08) between the declaration of AEs and the declared probability of suffering an error. The declaration of AEs doesn’t seem to relate (p=0,76) with the fact of seeing or listening recently to any news about an incident in healthcare (in the press, television, radio or internet). The patients that feel very safe show a higher level of information/satisfaction than the rest of patients (with an average of 95,23 (±7,89) compared to a 86,6 (±12,15) for the quite safe and 80,6 (±15,7) for the neither safe or unsafe). Only in the 36% of the cases have professionals detected an AE also declared by the patients, the proportion of concordance of positive identification by patients and professionals is of 12%. Having analyzed the agreement in detail, between the specific declaration of AEs by the patients and the declaration of the same by professionals, we find that in all the cases the degree of agreement is discreet and even without accordance (k≤0,4). We conclude that our low agreement hypothesis is confirmed and that it provides data against the suggestion to the scientific community regarding the reliability of the patients as identifiers of AEs, in its clinic conception.
