Abstract

Esta tesis doctoral titulada: “Ácido tranexámico en cirugía protésica de rodilla“del doctorando Xavier Aguilera Roig, es el compendio de una serie de estudios de investigación clínica, que incluye 7 publicaciones realizadas durante 4 años, del 2011 al 2015. La artroplastia total de rodilla es una de las cirugías más habituales en los quirófanos de cirugía ortopédica. En la última década ha habido un incremento significativo en el número de prótesis implantadas como resultado del aumento de las expectativas de vida y la edad de los pacientes, así como del aumento de pacientes jóvenes intervenidos. Todo esto ayudado por la mejora en las técnicas utilizadas, la adquisición de mayor experiencia por parte del cirujano y una mayor supervivencia del implante. No obstante, no es un procedimiento exento de complicaciones. El principal objetivo en el postoperatorio precoz en cirugía de artroplastia total de rodilla es la rápida recuperación del paciente. El resultado clínico de la artroplastia total de rodilla depende entre otros factores del diseño y el material de los componentes de la artroplastia total de rodilla, de la correcta colocación de los implantes, de las condiciones previas del paciente, y también del control del sangrado postoperatorio. No hay duda de que este último es uno de los factores que más influyen en su resultado inmediato. El sangrado postoperatorio de la artroplastia de rodilla puede variar entre 800 y 2000 mL, siendo necesario en muchas ocasiones las transfusiones de sangre alogénica. Los trabajos presentados en esta memoria pretenden obtener evidencias sobre la eficacia y seguridad del ácido tranexámico como fármaco antifibrinolítico, en pacientes intervenidos de artroplastia total de rodilla. La importancia de las publicaciones radica en la necesidad de desarrollar un adecuado control del sangrado y de aumentar la calidad y seguridad del paciente intervenido reduciendo además el coste de estos procedimientos quirúrgicos. El control óptimo del sangrado en el postoperatorio evita complicaciones propias de la cirugía protésica, así como las complicaciones relacionadas directamente con la anemia y transfusiones de sangre alogénica. La hipótesis del trabajo común entre las diferentes publicaciones presentadas en la memoria es: “el ácido tranexámico disminuye el sangrado postoperatorio en pacientes intervenidos de artroplastia total de rodilla, sin que se asocie a complicaciones”. Y el objetivo principal de la tesis fue valorar la eficacia y seguridad del ácido tranexámico en relación con el sangrado postoperatorio comparándolo con el tratamiento habitual de hemostasia habitual y con otros fármacos en artroplastia total de rodilla. Del las publicaciones presentadas se concluye que el ácido tranexámico ha demostrado ser eficaz y seguro para el control del sangrado en cirugía de la artroplastia total de rodilla. Asimismo, la memoria plantea futuros trabajos que complementan su línea de investigación.


This doctoral thesis entitled: "tranexamic acid in knee replacement surgery" of the candidate Xavier Aguilera Roig, is the epitome of a series of clinical research studies, including 7 publications over 4 years, from 2011 to 2015. Total knee replacement is one of the most common surgeries in orthopaedic surgery. In the last decade there has been a significant increase in the number of implanted prosthesis as a result of increased life expectancy and patient’s age and the increase of young patients undergoing this surgery. All this helped by the improvement in the techniques used, acquiring more experience by surgeons and increased implant survival. However, it is not a free process complication. The main objective in the early postoperative surgery of total knee arthroplasty is the fast patient recovery. The clinical outcome of total knee arthroplasty depends among other factors of the total knee arthroplasty component’s design and material, the correct placement of the implants, of the preoperative patient status and also the control of the postoperative bleeding. There is no doubt that the blood lost is one of the factors that influence its immediate result. The postoperative bleeding in knee arthroplasty varies between 800 to 2000 mL, being necessary in many cases allogeneic blood transfusions. The publications presented here seek to obtain evidence about the efficacy and safety of tranexamic acid as an antifibrinolytic drug, in patients undergoing total knee arthroplasty. The importance of these publications is the need to develop an adequate control of bleeding and increase the quality and safety of the surgical patient while reducing the cost of these surgical procedures. The optimal control of postoperative bleeding prevents own prosthetic surgery complications and complications directly related to anemia and allogeneic blood transfusions. The common hypothesis of the different publications presented in this memory is "tranexamic acid reduces postoperative bleeding in patients undergoing total knee arthroplasty, without associated complications." And the main aim of the thesis was to evaluate the efficacy and safety of tranexamic acid in relation to postoperative bleeding compared to standard treatment of routine hemostasis and other drugs in total knee arthroplasty. The publications submitted is concluded that tranexamic acid has been shown to be effective and safe for controlling bleeding in surgery after total knee arthroplasty. Also, the memory raises future studies to complement their line of research.
