Abstract

La heminegligencia visuo-espacial izquierda es un trastorno neurológico grave que afecta a dos tercios de los pacientes que han sufrido un ictus en la arteria cerebral media o posterior del hemisferio derecho. Es un déficit que interfiere de forma significativa en las actividades de vida diaria de los pacientes (vestido, higiene personal, alimentación, movilidad, entre otros); también se ha asociado con estancias hospitalarias más prolongadas y períodos de rehabilitación más largos. Frente a esta problemática, se han propuesto múltiples técnicas para aliviar, reducir o rehabilitar la heminegligencia visuo-espacial, no obstante, a día de hoy aún no se ha identificado un tratamiento plenamente eficaz. Los objetivos del presente trabajo fueron: 1) analizar si la administración de intervenciones combinadas, en pacientes que presentaban heminegligencia visuo-espacial izquierda, era más eficaz que la aplicación aislada de un único tratamiento; 2) estudiar la relación entre el tiempo de evolución post-ictus y la respuesta a dos tipos de tratamiento, único y combinado. El tratamiento único consistió en realizar ejercicios de rehabilitación cognitiva informatizada mediante la plataforma de telerehabilitación Guttmann, NeuroPersonalTrainer®; el tratamiento combinado estuvo compuesto por la realización de los mismos ejercicios junto con un dispositivo visual que llevaba el hemicampo derecho de cada ojo ocluido (técnica del right hemifield eye patching). El protocolo de exploración neuropsicológico para los tres estudios estuvo formado por: Bells Test, Figure Copying of Ogden, Line Bisection, Baking Tray Task y Reading Task. La repercusión funcional del tratamiento se valoró mediante The Catherine Bergego Scale. En el primer estudio 12 pacientes fueron distribuidos aleatoriamente en dos grupos de tratamiento: tratamiento único (n=7) y tratamiento combinado (n=5). En ambos casos, el tratamiento comprendió una media de 15 sesiones de 1 hora de duración. Los resultados mostraron diferencias intergrupales estadísticamente significativas en la comparación de las diferencias post-pre tratamiento en la prueba Reading Task. No se obtuvieron diferencias funcionales en The Catherine Bergego Scale. En el segundo estudio se mantuvo la estructura y la metodología del anterior con la finalidad de comprobar si el efecto obtenido, en el primer estudio, se mantenía o aumentaba con un tamaño muestral mayor. Se realizó un ensayo clínico aleatorizado realizado con 28 pacientes compuesto por dos grupos experimentales: el grupo que recibió el tratamiento único (n=15) y, el grupo que recibió el tratamiento combinado (n=13). Todos ellos también realizaron también 15 sesiones de 1 hora de duración de rehabilitación cognitiva informatizada. Tras el tratamiento, los dos grupos mostraron cambios significativos a nivel intragrupal en el protocolo de exploración neuropsicológico aunque no se obtuvieron diferencias intergrupales, ni en el protocolo de exploración ni en la escala funcional. En el tercer estudio 31 pacientes fueron aleatorizados en dos grupos de tratamiento: tratamiento único (n=18) y, tratamiento combinado (n=13). En esta ocasión, cada grupo de tratamiento fue dividido en función del tiempo que había transcurrido entre el ictus y el inicio del tratamiento (≤ 12 semanas vs. > 12 semanas). Los resultados indicaron cambios a nivel intragrupal en los cuatro grupos, sin embargo, no se obtuvieron diferencias intergrupales. Ni el tiempo transcurrido entre el ictus y el inicio del tratamiento, ni el tipo de tratamiento recibido, parecieron ser variables que influyan en la mejora de estos pacientes.


Visuo-spatial neglect is a serious neurological disorder that affects two thirds of patients who have suffered a stroke in the middle or posterior cerebral artery of the right hemisphere. It is a deficit that can significantly interfere with patient´s activities of daily living (dressing, personal hygiene, feeding, mobility, etc.). It has also been associated with larger hospital stays and longer periods of rehabilitation. For this reason, many techniques have been proposed to alleviate, reduce or rehabilitate visuo-spatial neglect, however, nowadays effective treatment has not yet been identified. The aims of this work were 1) to analyze whether the administration of combination treatment in patients with visuo-spatial neglect, was more efficacious than single treatment; 2) to study the relation between post-stroke time evolution and to analyze the patient´s response to two treatments, single treatment vs. combination treatment. The single treatment groups followed a cognitive rehabilitation program using the computer-based platform Guttman, NeuroPersonalTrainer®; the combination treatment group received the same cognitive rehabilitation program but in combination with a visual device incorporated into the right hemifield eye patching, in which the right hemifield of each lens was completely opaque. Visuo-spatial neglect was assessed using a specific exploration protocol (Bell Cancellation Test, Figure Copying of Odgen, Line Bisection, Baking Tray Task, and Reading Task). The functional effects of the treatment were assessed using the Catherine Bergego Scale. In the first study, 12 patients were randomized into two treatment groups: a single treatment group (n= 7) and a combination treatment group (n= 5). In both cases, the treatment consisted of a mean number of 15 sessions each lasting 1 hour. Significant between-group differences were observed when comparing the pre- and post-treatment scores for the Reading Task. No differences were observed in either group in the Catherine Bergego Scale administered at baseline and at the final intervention. In the second study the structure and methodology was maintained from the previous study in order to check whether the effect obtained was maintained or increased with increasing sample size. A randomized clinical trial with 28 patients composed of two experimental groups was performed: single treatment group (n= 15) and combination treatment group (n= 13). In both cases, the treatment consisted of a mean number of 15 sessions each lasting 1 hour. After treatment, both groups showed significant changes intragroup, but did not show differences between-group in the exploration protocol or the functional scale. 31 patients were randomized into two treatment groups: a single treatment group (n=18) and a combination treatment group (n=13). In both cases, the treatment consisted of a mean number of 15 sessions each lasting 1 hour. Each treatment group was divided according to the period that elapsed between the stroke and the start of treatment (≤ 12 weeks vs. > 12 weeks). The results obtained after applying these treatments indicated changes at intragroup level in the four groups analyzed. However, no between-group differences were found. No differences in either group were observed in the Catherine Bergego Scale. Neither the time elapsed between the stroke and the beginning of treatment nor the type of treatment received appear to be variables that influence the improvement of these patients.
