Abstract

La infección por el virus hepatitis C (VHC) y por el virus de la Inmunodeficiencia humana tipo I (VIH) constituyen un importante problema de salud a nivel mundial. Los niños hijos de madres infectadas por el VIH están expuestos a combinaciones de fármacos antirretrovirales (ARV) durante el período prenatal y perinatal. A pesar del beneficio obtenido del uso de ARV para la prevención de la transmisión vertical, se desconoce el impacto que pueden tener en el en el desarrollo neuropsicológico de los niños sanos expuestos a ARV. En el caso del VHC, el riesgo de transmisión vertical es mucho menor. A pesar de que están disponibles nuevos antivirales para curar la infección están contraindicados durante el embarazo. Las madres de estos pacientes comparten características sociodemográficas y antecedentes de consumo de tóxicos durante el embarazo. Son escasos los estudios en pacientes pediátricos sanos hijos de madres con infección por el VIH o por el VHC que valoren el funcionamiento cognitivo global y específico y el rendimiento escolar de esta población de niños. Objetivo. Describir el funcionamiento cognitivo y conductual global en una población de niños sanos expuestos prenatalmente a VIH o VHC a los 6-8 años y su relación con las variables relacionadas con la gestación y nacimiento y las variables socioeconómicas y sociodemográficas. Determinar el impacto del funcionamiento neuropsicológico en el rendimiento escolar cuatro años después de la valoración neuropsicológica. Sujetos y métodos. Se incluyeron 115 niños sanos nacidos de madres infectados por el VIH (78,3 %) o por el VHC (21,3%). Se realizó una exploración neurocognitiva amplia evaluando tanto el funcionamiento cognitivo global como el funcionamiento cognitivo específico. Se administraron cuestionarios de conducta a padres y se valoró el rendimiento académico a los 4 años de realizar la exploración neuropsicológica. Se registraron una serie de variables clínicas relacionadas con la gestación y nacimiento y con las variables socioeconómicas y sociodemográficas. Para estudiar la adecuación de las agrupaciones factoriales propuestas se llevó a cabo un análisis factorial confirmatorio (AFC). Se utilizó como método de clasificación el análisis de clases latentes (ACL). Adicionalmente, permitió incluir covariables. Resultados. Los sujetos de la muestra de estudio siguen una distribución normal en cuanto a los resultados del cociente intelectual. En cuanto al funcionamiento cognitivo específico, una vez eliminados los tres sujetos con puntuaciones inferiores a 70, identificamos tres clústeres de pacientes: perfil cognitivo alto (PCA) (17,9%), perfil cognitivo medio (PCM) (57,1 %) y perfil cognitivo bajo (PCB) (25%). El PCB tenía un perfil heterogéneo evidenciando resultados más bajos en lenguaje, visoconstrucción y en algunas Funciones ejecutivas (control atencional y establecimiento de objetivos), además de presentar más síntomas de conducta externalizante. La probabilidad de tener dificultades escolares a los 4 años es significativamente diferente según el clúster. La probabilidad es de cero para los niños del PCA y del 7% en aquellos de PCM pero se dispara hasta el 30% en los de PCB. En el análisis multivariante, los niños procedentes de familias con un bajo nivel económico tienen una probabilidad 31 veces mayor (odds ratio 31,09); IC 95%: 2,52-382,89) de pertenecer al PCB vs al PCA y 11 veces mayor (odds ratio 11,37; IC 95%: 2,00-65,78) de pertenecer al PCB vs al PCM. Conclusiones.La exposición a fármacos antirretrovirales durante la gestación y el periodo neonatal no determina una alteración en el funcionamiento neurocognitivo. Una situación socioeconómica baja es un factor de riesgo de presentar dificultades en funciones cognitivas específicas, más problemas de conducta externalizante y más dificultades en el rendimiento académico.


Infection with the hepatitis C virus (HCV) and infection with the human immunodeficiency virus type I (HIV) are a major health problem worldwide.Children of mothers infected with HIV are exposed to combinations of antiretroviral drugs (ARVs) during the prenatal and perinatal periods.In spite of the benefits derived from the use of ARVs in order to prevent vertical mother-to-child transmission, the impact of exposure to the ARVs on the neuropsychological development in healthy children is still unknown. In the case of HCV, the risk of vertical transmission is much lower. Even when new antivirals are currently available as a cure for this type of infection, they are contraindicated for use during pregnancy. Mothers of these patients share sociodemographic characteristics and a history of drug and alcohol abuse during pregnancy. There are very few studies about healthy paediatric patients from mothers with HIV or HCV to assess both the overall and specific cognitive functioning, aswell asthe school performance of this population of children. Objectives • To describe the overall cognitive and behavioural functioning within a population of healthy children aged 6-8 who had been prenatally exposed to HIV or HCV, and their relationship with the variables related to pregnancy and birth, as well as to their socioeconomic and sociodemographic variables. To determine the impact of neuropsychological functioning in these children’s school performance 4 years after the neuropsychological assessment. Subjects and methods This study included 115 healthy children born from mothers infected with HIV (78,3 %) or HCV (21,7%). A wide neurocognitive examination was conducted to evaluate both the global cognitive functioning and the specific cognitive functioning. Behavioural questionnaires were administered to parents, and the academic performance of these children was assessed 4 years after the neuropsychological examination had been conducted. Besides, sociodemographic, socioeconomic and gestational variables were collected. A confirmatory factor analysis(CFA) was also conducted in order to study the adequacy of these factor groups. Latent class analysis(LCA) was used as a method of classification. Additionally, the LCA afforded the opportunity to include covariates. Results As far as the global cognitive functioning is concerned, the subjects studied follow a normal distribution throughout the results of the IQ test. Concerning their cognitive functioning, we were able to identify three clusters of patients: 17% with a high cognitive profile (HCP), 57,1% with a medium cognitive profile (MCP) and 25% with a low cognitive profile (LCP). The LCP cluster showed a heterogeneous profile, displaying lower scores on language tests, in visuoconstruction and in some executive functions (attention control and goal setting), in addition to presenting more symptoms of externalizing behaviour.The probability for these 4-year-old school children to have academic difficulties is significantly different depending on each cluster. There is a zero probability for HCP children and a 7% probability for MCP children, but the percentage shoots up to 30% in LCP children. On a multivariate analysis, children from lower socioeconomic families have a 31% larger probability (odds ratio 31,09); IC 95%: 2,52-382,89)of belonging to LCP vs. HCP groups. Socioeconomic status was the only risk factor identified within the LCP group. Similarly, The probability of belonging to LCP vs. MCP groups is estimated to be 11 times larger (odds ratio 11,37; IC 95%: 2,00-65,78). Conclusions The safety of prenatal and neonatal ARV use has been supported. Low socioeconomic status is a risk factor for presenting difficulties in specific cognitive functions and externalizing behaviour problems. Subjects included in this group present greater difficulties in their academic performance.
