Abstract

Presentamos un estudio prospectivo, realizado sobre un total de 144 pacientes intervenidos de desprendimiento rhegmatógeno primario de retina (DRR), con un seguimiento mínimo de la muestra de 12 meses. En este trabajo se presenta un sistema de clasificación del volumen de taponador intraocular que permite determinar de forma reproducible y sistemática el volumen de gas existente en la cavidad vítrea en el periodo postoperatorio. Este sistema permite unificar los criterios existentes entre los diferentes especialistas, así como analizar los factores que condicionan el volumen de agente taponador en el postoperatorio inmediato de los pacientes intervenidos de vitrectomía pars plana (VPP). Según nuestros resultados, los factores que determinan el volumen de agente taponador intraocular serían el estado del cristalino y el número de cuadrantes de disección del gel vítreo durante la VPP. En el presente estudio se analiza la correlación entre los cambios estructurales en la retina externa y la función visual tras conseguir el éxito anatómico en pacientes intervenidos de DRR primario con mácula off mediante VPP. Nuestros resultados sugieren que la integridad de la microestructura foveal, el grosor foveal y el grosor coroideo subfoveal parecen influir en la función fisual en pacientes con DRR primario con mácula off. Además, en este trabajo hemos estudiado un nuevo concepto: la velocidad de restauración de la anatomía de la retina externa, encontrando una elevada asociación entre esta variable y la agudeza visual final de los pacientes con DRR primario mácula off. Este hallazgo refuerza la idea de que la función visual no sólo estaría condicionada por la integridad o alteración de la retina externa; sino que también se asocia a la velocidad de restauración de la misma.


The design of this study is a prospective, consecutive, observational case series of 144 patients with primary rhegmatogenous retinal detachment (RRD), with a minimun follow-up of 12 moths. We describe a new classification system of the intraocular tamponade volume, which allows a systematic determination of the gas volume in the vitreous cavity following pars plana vitrectomy (PPV). This classification pretends to unify the different criteria of the retinal specialists, as well as serve as a tool to analyse the different factors that determine the volume of the gas tamponade in the vitreous cavity following PPV. In the present series, we identify the status of the crystalline lens (phakic vs pseudophakic) and the extension of the vitrectomy (measured as the number of quadrants of the vitreous base dissected during PPV) as the main factors that determine the volume of the gas tamponade in the vitreous cavity following PPV. This report evaluates the correlation between serial microstructural changes in the outer retina and visual function after successful repair of primary rhegmatogenous retinal detachment (RRD) with macula off. Integrity of the foveal microstructure, foveal thickness, and subfoveal choroidal thickness may account for visual restoration in preoperative macula off retinal detachment patients. Furthermore, in the present report we describe the photoreceptor layer restoration speed; and find an intense correlation between this variable and the final best corrected visual acuity (BCVA) of these patients. This new finding suggests that visual function not only depends on the integrity or alteration of the foveal microstructure, but is also associated to the speed of restoration of this structure. Photoreceptor layer restoration speed may be used as a new predictor of final BCVA in these patients.
