El gen precore-core del virus de la hepatitis B (VHB) se transcribe en dos ARN diferentes (ARNcore y ARNprecore) por la presencia en el promotor básico para este gen (PBC) de elementos que dirigen independientemente la síntesis de estos dos ARN. En la región precore del mismo gen se han descrito mutaciones que impiden la síntesis del HBeAg. Objetivos: 1.Estudiar en pacientes con hepatitis crónica B y portadores asintomáticos (p.a.) del VHB las concentraciones de ADN-VHB y HBeAg, el genotipo viral y la lesión histológica hepática. Relacionar con la presencia de mutaciones en el PBC y precore. 2. En pacientes con infección por VHB, VHC y/o VHD, determinar la concentración de ADN-VHB, ARN-VHC y ARN-VHD, genotipo viral y distribución de mutaciones en el PBC y precore del VHB. Pacientes: Objetivo 1: 182 HBsAg(+):116 hepatitis crónica: 30 HBeAg(+) con ALT elevada (14 HCA y 2 CH), 58 HBeAg(-) con ALT elevada (24 HCA y 10 CH), 28 ALT fluctuante (ALTf): 27 HBeAg(-) y 1 HBeAg(+) (6 HCA); 66 p.a. HBeAg(-). Objetivo 2: 175 Hepatitis crónica: 65 Coinfecciones (25 VHB/VHC, 18 VHB/VHD y 22 VHB/VHC/VHD) y 110 Infección aislada (55 VHB y 55 VHC). Métodos: VHB: HBsAg (EIA), HBeAg/Anti-HBe (RIA), cuantificación del HBeAg (RIA), determinación cualitativa del ADN-VHB (doble PCR), cuantificación del ADN-VHB (PCR en tiempo real), genotiopado VHB y mutaciones en PBC y precore (doble PCR y secuenciación). VHC: Anti-VHC (EIA), determinación cualitativa del ARN-VHC (RT-PCR), cuantificación del ARN-VHC (monitor test), genotipado VHC (RT-PCR-DEIA).VHD: Anti-HD (RIA), determinación cualitativa del ARN-VHD (RT-PCR), genotipado VHD (secuenciación). Resultados y conclusiones: 1. Mutaciones en el PBC en nuestra área: 72% en la hepatitis crónica HBeAg(-), 47% en hepatitis crónica HBeAg(+) y 52% en p.a. 2. La mutación mayoritaria en el PBC fue la sustitución de una G por una A en la posición 1764 (95% de las secuencias mutadas): su análisis sería el más sensible y suficiente para el estudio global del PBC. 3. Los pacientes con mutaciones en el PBC presentaron menor concentración de HBeAg: mutaciones en el PBC disminuyen pero no anulan la expresión del HBeAg; pero la presencia de estas mutaciones no afectaba a la concentración de ADN-VHB. 4. Las variantes de la región precore que impiden la expresión del HBeAg se detectaron preferentemente en la hepatitis crónica HBeAg(-) y, en proporción inferior, en p.a. Las variantes más frecuentes: posiciones 1896 (73% de las secuencias mutadas) y 1814-1817 (23% de las secuencias mutadas); para el estudio de la región precore es necesario realizar el análisis de estas mutaciones. 5. Análisis conjunto del PBC y precore: únicamente la presencia de mutaciones en las posiciones 1896 y 1899 del precore está asociada con mayores concentraciones de VHB. Las mutaciones en el PBC y/o precore están relacionadas con el estadío HBeAg(-) y la cirrosis hepática (posiciones 1764, PBC, y 1899, precore) y permite diferenciar a nivel molecular la hepatitis crónica ALTf del p.a. del VHB. 6. El genotipo A, mayoritario en el grupo HBeAg(+) con ALT elevada, y el genotipo D en HBeAg(-) con ALT elevada. La prevalencia de ambos genotipos era similar en ALTf y p.a. La presencia de mutaciones en el PBC se relacionó con el genotipo A mientras que en el precore con el genotipo D. En cada grupo de pacientes la carga viral era independiente del genotipo. 7. En las coinfecciones por VHB y VHC se observó una inhibición mutua entre ambos, con un mayor efecto inhibidor del VHC sobre el VHB. En la coinfección triple, el VHD actúa como dominante, e inhibe mayormente al VHC. La interacción entre los distintos virus no está influida por los genotipos. La coinfección por el VHC se relacionó con menor proporción de mutaciones en el PBC y precore del VHB.The precore-core gen of the (HBV) is transcribed in 2 differents mRNAs (RNAcore and RNAprecore) by the action of differents elements in the basic core promoter for this gen (BCP). In the precore region of the same gen have been showed mutations able to abolish the synthesis of HBeAg. Aims: 1.To stablish in patients with chronic hepatitis B and HBV assymptomatic patients (AP), HBV-DNA and HBeAg concentrations, HBV genotype and the liver histologycal injury. Relation with mutations in BCP and precore. 2. To stablish in patients with HBV, HCV and/or HDV infection, the HBV-DNA, HCV-RNA and HDV-RNA concentrations, viral genotype and the presence of BCP and precore mutations. HCV: Anti-HCV (EIA), HCV-RNA qualitative determination (RT-PCR), HCV-RNA quantification (monitor test), HCV genotype (RT-PCR-DEIA).HDV: Anti-HD (RIA), HDV-RNA qualitative determination (RT-PCR), HDV genotype (sequenciation). Results and conclusions: 1. BCP mutations in our area: 72% in HBeAg(-) chronic hepaititis, 47% in HBeAg(+) chronic hepatitis and 52% in AP. 2. The more prevalent mutation in BCP was the substitution of G to A in 1764 (95% in BCP mutated sequences): this analysis would be sufficient to the overall study of BCP. 3. Patients with BCP mutations presented a lower HBeAg concentration: BCP mutations reduce but not abolish HBeAg expression; nevertheless these mutations ware not related with HBV-DNA levels. 4. The precore region variants that abolish HBeAg expression was detected preferably in HBeAg(-) chronic hepatitis and, in a lower proportion, in AP. The more frequent variants: positions 1896 (73% in precore mutated sequences) and 1814-1817 (23% in precore mutated sequences): to study the presence of precore mutations is necessary to performance the analysis of these two regions. 5. Combined analyses of BCP and precore: only the presence of mutations in 1896 and 1899 was related with highers HBV-DNA concentrations. BCP and/or precore mutations was related with the HBeAg(-) state and HC (1764 position -BCP- and 1899 - precore-) and allow the differenciation between ALTf and AP in a molecular level. 6. Genotype A was found more frequently in HBeAg(+) with rised ALT, and genotypo D in HBeAg(-) with elevated ALT. The frequency of these genotypes was similar in ALTf and AP. BCP mutations were related with genotype A and precore mutations with genotypo D. In each group of patients HBV-DNA leves ware not associated with genotype. 7. A mutual nhibitory action between HBV and HCV in this dual coinfection was showed, with a stronger inhibitory effect of HCV over replication of HBV. HDV had a dominant role in triple coinfection, and seemed to exert a higher negative influence on HCV replication than on HBV. The interaction between the differents virus was not affected by the genotypes. HCV coinfection was related with a lower proportion of patients with BCP and precore mutants.
