Debido al aumento del número de nacimientos prematuros y a las importantes secuelas que éstos conllevan y, por otra parte, a las investigaciones sobre los efectos potencialmente dañinos de las unidades de cuidados intensivos neonatales (UCIN), resulta cada vez más necesario disponer de una herramienta que permita evaluar a los recién nacidos prematuros ingresados en una UCIN sin añadir ningún elemento disruptivo y permitiendo generar un programa de intervención posterior. El Newborn Individualized Developmental Care and Assessment Program (NIDCAP) incluye una Pauta de Observación que se adecua perfectamente al entorno de una UCIN y permite generar un programa de cuidados posterior. No obstante esta técnica no se ha usado a nivel cuantitativo y no se tienen datos sobre sus propiedades psicométricas. Esta Tesis se plantea con el objetivo de comprobar la sensibilidad y el valor predictivo de la Pauta NIDCAP.
En este trabajo se ha realizado el seguimiento de una muestra de 45 recién nacidos prematuros desde su nacimiento hasta los nueve meses de edad gestacional (EG). Se han evaluado los niños cuatro veces: al nacimiento y a las 34 semanas de EG con la Pauta NIDCAP, a las 40 semanas de EG con la Escala de Brazelton (NBAS) y a los nueve meses de EG con las Escalas de Desarrollo Bayley-III (BSID-III).
Al analizar los resultados se ha observado una variabilidad de las conductas entre el nacimiento y las 34 semanas de EG, tanto cuando se considera todo el grupo en conjunto como cuando se considera dividido en dos en función de la EG de nacimiento. Se ha establecido un patrón conductual a las 34 semanas de EG a partir de las conductas de la Pauta NIDCAP con mayor tasa de ocurrencia a esa edad. Se ha observado que la Pauta NIDCAP se correlaciona tanto con la NBAS como con las BSID-III. Se han encontrado diferencias entre las conductas de la Pauta NIDCAP a las 34 semanas de EG de prematuros con y sin un desarrollo normal a los nueves meses de EG según las BSID-III.
La Pauta NIDCAP es sensible al paso del tiempo, a la madurez evolutiva y al sexo de los recién nacidos evaluados. El valor predictivo de la Pauta NIDCAP viene dado, a corto plazo, por sus capacidades de realizar un pronóstico neuroconductual para las 40 semanas de EG y de llevar a cabo una intervención con los padres y, a largo plazo, por detectar señales de alerta a las 34 semanas de EG. La presencia de señales de estrés a las 34 semanas de EG indica la necesidad de realizar una intervención más específica en función del tipo de deterioro en el desarrollo.
Due to the increasing number of preterm births and to the important sequels that they comport and due also to researches on the potentially harmful effects of neonatal intensive care units (NICU), it becomes more important to have an assessment tool for the preterm newborns hospitalized in a NICU that does not bring in any disruptive element and makes it easier for clinical staff to generate an intervention program. The Newborn Individualized Developmental Care and Assessment Program (NIDCAP) provides an Observation Checklist, adequate to be applied in a NICU, that can be used to generate a subsequent care program. Nevertheless, this technique has never been used as quantitative analysis method and no data are available about its psychometric characteristics. The aim of this thesis is to determine the sensitivity and the predictive value of the NIDCAP Checklist.
In this research a group of 45 preterm newborns has been followed up from their birth to nine months of gestational age (GA). The newborns have been assessed four times: at birth and at 34 weeks GA using the NIDCAP Checklist, at 40 weeks GA using the Brazelton Scale (NBAS) and at nine months of GA with the Bayley-III Scales (BSID-III).
Analysis of the results reveals the existence of considerable variability in the NIDCAP behaviours between the birth assessment and the assessment at 34 weeks GA. The variability is observed when the entire sample is considered as well as when two sub-sets, with different GA ranges, are studied. A behaviour pattern has been established at 34 weeks GA using the most frequent NIDCAP behaviours. NIDCAP-NBAS and NIDCAP-BSID-III correlations have been found. When preterm sample is split in two sets according to the impairment in development at nine months, using BSID-III, significant differences in the NIDCAP behaviours have been found between the two sets.
The NIDCAP Checklist is sensitive to GA differences, psychological maturity, and sex of newborns. The short term predictive value of the NIDCAP Checklist implies its ability to be a good basis for a neurobehavioral prognosis at 40 weeks GA and its ability to be an accurate intervention tool for parents. The long term predictive value of NIDCAP is based on its ability to detect alert signs at 34 weeks GA. The presence of these alert signs reveals a need to carry out more specific intervention depending on the kind of impairment in development.
