El càncer colorectal és el tipus de neoplàsia més comuna en els països desenvolupats i representa la segona causa de mort per càncer. Per tant, el coneixement de les bases moleculars implicades en la carcinogènesi poden facilitar el diagnòstic i el tractament del càncer.
Un dels gens implicats en les bases moleculars del càncer de còlon és el proto-oncogèn K-RAS. El gen K-RAS es localitza en el cromosoma 12 i codifica per la proteïna K-RAS. El proto-oncogèn K-RAS s’activa específicament per mutacions puntuals en el codó12 (aproximadament el 82% de totes les mutacions) i el codó13 (aproximadament el 17%) de l’exó 2 del gen K-RAS. S’han descrit altres mutacions en el codó 61 i 146, representant aproximadament entre 1-4 %. En el càncer colorectal aquestes alteracions es presenten en aproximadament el 40% dels casos. Aquestes mutacions provoquen canvis en l’activitat GTPasa i ocasionen acumulació de la proteïna RAS en el seu estat actiu (unida a GTP). La proteïna mutada altera les vies de trasducció de senyals mitogèniques produint-se l’estimulació del creixement cel·lular i l’expansió clonal.
L’estadiatge del càncer colorectal es basa en una sèrie de criteris clinicopatològics que es tenen en compte per l’elecció del tractament al qual es sotmetrà el pacient. Pels tractaments actuals es necessita conèixer les bases biològiques i moleculars dels tumors per poder oferir el tractament més correcte, i per tant, es necessita conèixer els factors predictius i els factors pronòstic de cada pacient.
Nombrosos estudis han determinat el valor predictiu de l’estatus mutacional de K-RAS davant la resposta al tractament amb cetuximab (anticòs monoclonal dirigit contra el domini extracel·lular de la proteïna de membrana EGFR), sobretot en pacients amb càncer colorectal metastàtic. No obstant, el valor de les mutacions en K-RAS com a factor pronòstic en càncer de còlon encara està per confirmar, ja que els estudis actuals són controvertits en aquest aspecte.
En aquesta tesi, ens vam marcar uns objectius que eren: determinar la presència de mutacions en el gen K-RAS en mostres tumorals de còlon, valorar la supervivència global i el temps lliure de malaltia en relació a la presència o no d’aquestes mutacions i valorar si la mutació de K-RAS es comporta com un factor pronòstic independent.
S’ha estudiat la presència de mutacions en el gen K-RAS en 235 pacients diagnosticats de càncer colorectal durant el període comprès entre 2004-2005 en el Hospital Universitari Mútua Terrassa.
Per l’estudi de les mutacions de K-RAS s’ha utilitzat el Kit Therascreen (DxS Diagnostics) aprovat per la Unió Europea amb la tecnologia de PCR-RT, amb sondes fluorescents tipus Scorpion (Amplification refractory mutation system ARMS).
De cada pacient s’han recollit unes dades clinicopatològiques: edat, sexe, data cirurgia, localització del tumor, tipus histològic, grau histològic, invasió limfàtica, invasió venosa intramural i extramural, invasió perineural, pT, pN, CEA, quimioteràpia neoadvujant, quimioteràpia adjuvant, recidiva local, metàstasis pulmonars, metàstasis hepàtiques o en altres localitzacions, quimioteràpia metàstasis, tractament amb cetuximab, èxitus, situació actual, última revisió oncològica i l’estat mutacional de K-RAS.
S’han utilitzat el mètode de Kaplan-Meier per estimar les corbes de supervivència respecte l’estat muracional de K-RAS (mutat vs no mutat), respecte el temps de supervivència global i el temps lliure de malaltia. S’han utilitzat models multivariables de regressió de Cox per estimar els quocients de risc associats amb KRAS (natiu vs mutat) i valorar si les mutacions en KRAS es comporten com un factor pronòstic independent.Colorectal cancer is one of the most common human malignances and one of the leadingd causes of cancer-related deaths in the development countries. Improvements in the knowledge of cancer biology have led to the development of the best diagnostic and teatment.
The ras family of oncogens comprise three principal members (K-RAS, H-RAS and N-RAS) all witch have been implicated in the development of human maliganances. The K-RAS oncogene located on chromosome 12p12 encodes a 21 kD potein involved in the G-protein signal transduccion pathway, modulating cellular proliferation and diferentiation.
Oncogenic mutations of the K-RAS genes are observed in about 40% (20–50%) of sporadic colorectal cancers. These are point mutations and are generally observed as somatic mutations. Up to 90% of activating mutations of the RAS gene are detected in codons 12 (∼82%) and 13 (∼17%). The most frequent types of mutations of the K-RAS oncogene result in constitutive activation of this signal transduction pathway and, consequently, unregulated proliferation and impaired differentiation.
The staging of colorectal cancer based on a series of clinical-criteria taken into account by the choice of treatment to which the patient is subjected. For current treatments will need to know the biological and molecular tumor to provide the correct treatment and, therefore, we need to know predictive factors and prognostic factors of each patient.
The EGFR has become an important target in the treatment of metastatic colorectal cancer. Mutations of the K-RAS gene may activate downstream signal transduction and confer resistance to upstream inhibition of the EGFR by monoclonal antibodies. The K-RAS mutation status has been identified as a strong predictor of response to anti-EGFR antibodies in clinical trials agents such as cetuximab or panitumumab should, therefore,only be offered to patients with a K-RAS wild-type.
Measurement of K-RAS oncogene abnormalities has not been demonstrated to be an independent predictor for either survival, quality of life,or disease-free survival in patients with large bowel carcinoma.
Abnormalities of K-RAS in colorectal tissue may correlate with increased relapse rate and decreased survival. However, currently available data do not demonstrate that ras oncogene analysis provides independent prognostic information in CRC.
In this thesis, we set some goals were: to determine the presence of mutations in K-RAS in colorectal tumor samples, assess overall survival and disease-free time in relation to the presence or absence of these mutations and assess whether the mutation of K-RAS acts as an independent prognostic factor.
Hematoxlin and eosin stained slides from 235 primary CRC patients with corresponding paraffin-embedded material available for molecular analysis were retrospectively collected from the Hospital Universitari Mútua Terrassa during de period 2004-2005.
The K-RAS mutation is detected with Therascreen (DxS Diagnostic), with PCR-real time, using ARMS (amplification refractory mutation system).
Histopathological and clinical data criteria were reviewed and included: age, gender, date surgery, tumor location, histologic grade, lymphatic invasion, venous invasion intramural and extramural, perineural invasion, pT and pN classification, CEA, chemotherapy neoadvujant, chemotherapy adjuvant, local recurrence, lung metastases, liver metastases and metastases in other locations, chemotherapy metastases, treatment with cetuximab, deaths, current, latest revision cancer and the mutational status of K-RAS.
Clinical outcome of interest was cancer-specific survival time, which was available for 235 patients.
Kaplan-Meier methods were used to estimate the survival curves by K-RAS status (wild-type vs mutated), with overal survival and disease-free time. Cox proportional hazards multivariable regression models were used to estimate the hazard ratios associated with K-RAS (wild-type vs mutated).
