Estudio comparativo entre la cirugía percutánea y la cirugía abierta en las rupturas agudas del tendón de aquiles

Las rupturas agudas del tendón de Aquiles son una lesión común y la incidencia está
aumentando. El tratamiento podría dividirse en cirugía conservadora, abierta y percutánea. Hay
muchos estudios en la literatura comparando los diferentes tratamientos, habiendo partidarios de
cada uno de ellos. La cirugía percutánea es una buena opción frente a la cirugía abierta, teniendo
menos complicaciones y una incidencia similar de rerupturas.
El propósito de esta tesis fue demostrar que el tratamiento percutáneo proporcionaba mejores
resultados funcionales, menos complicaciones y una recuperación más rápida que la cirugía
abierta.
El estudio preliminar en cadáver nos ayudó a minimizar la curva de aprendizaje y a evaluar la
fiabilidad y precisión de la técnica.
Hubo dos grupos en el estudio clínico, uno fue tratado con cirugía percutánea y el otro con cirugía
abierta. Los pacientes en este estudio prospectivo y aleatorizado fueron evaluados mediante
evaluación morfológica, clínica y funcional a los 3, 6 y 12 meses. También se evaluó la
dinamometría isocinética y el tiempo para volver al nivel de actividad previo.
En cuanto a la RM y la ecografía, los hallazgos más frecuentes fueron heterogeneidad y
engrosamiento del tendón, sin significado clínico. El adelgazamiento del tendón se asoció con
rerupturas y peores puntuaciones funcionales. Se produjo una reruptura en cada uno de los
grupos y una lesión parcial del nervio sural en el grupo percutáneo, sin diferencias significativas
entre ambos grupos. Hubo más complicaciones con la reparación percutánea, con significación
estadística. Hubo diferencias estadísticamente significativas en los siguientes ítems funcionales:
Achilles Tendon Rupture Score, Short-Form-36, dinamometría isocinética.
No hubo diferencias entre los grupos en el tiempo para volver al trabajo, o tiempo de
hospitalización.
A partir de los resultados obtenidos en el presente estudio podemos concluir que:
• La técnica percutánea utilizada en este trabajo no es absolutamente fiable en el estudio en
cadáver.
• En el presente estudio, no se han encontrado diferencias morfológicas significativas mediante
RM y ecografía entre ambos tratamientos, en cuanto a calidad cicatricial y grosor tendinoso.
• Las complicaciones globales son más frecuentes en el grupo de cirugía percutánea.
• En general, los resultados funcionales son similares en ambos grupos de tratamiento.
• No encontramos diferencias significativas en el tiempo requerido para regresar al trabajo
después de ambos tratamientos.
• No podemos recomendar uno u otro tratamiento y consideramos que dependerá de las
preferencias del cirujano y de la técnica con la que se sienta más cómodo, así como de las
circunstancias del paciente y sus aspiraciones deportivas. No se han encontrado diferencias
significativas en la evolución y no hay datos que apoyen que el tratamiento percutáneo es mejor
que el tratamiento por cirugía abierta.
Acute Achilles tendon ruptures are a common injury and the incidence is increasing. The
treatment could be grouped in conservative, open and percutaneous surgery. There are many
studies in the literature comparing the different treatments, and so, there are supporters of each
one of them. The percutaneous surgery is likely a good option in front of the open surgery,
because of less minor complications and similar incidence of reruptures.
The purpose of this thesis was to demonstrate that the percutaneous treatment provided better
functional results, less complications and a faster recovery than the open surgery technique.
Cadaver preliminary study help us to minimize learning curve and to evaluate the reliability and
precision of the technique.
There were two groups in the clinical study, one was treated with percutaneous surgery and the
other one, with open surgery. Patients in this prospective randomized study were evaluated by
means of morphological, clinical and functional evaluation at 3, 6 and 12 months. Isokinetic
dynamometry, and time to return to previous activity level also were evaluated.
Regarding MRI and ultrasound, the most frequent findings were heterogeneity and thickening of
the tendon without clinical meaning. Thinning of the tendon was associated with reruptures and
worse functional scores. One re-rupture occurred in each of the groups, and one partial sural
nerve injury occurred in the percutaneous group, without significant differences between both
groups. There were statistically significantly more complications with percutaneous repair. There
were statistically significantly differences in the following functional items: Achilles Tendon
Rupture Score, Short-Form-36, isokinetic dynamometry.
There were no differences between both groups in terms of time to return to work, or
hospitalization time.
From the results obtained in the present study we can conclude that:
• The percutaneous technique used in this work is not absolutely reliable in the cadaver study.
• In the present study, no significant morphological differences have been found using MRI and
ultrasound between both treatments, in terms of healing quality and tendon thickness.
• Global complications are more frequent in the percutaneous surgery group.
• Overall, the functional results are similar in both treatment groups.
• We did not find significant differences in the time required to return to work after both
treatments.
• We cannot recommend one or the other treatment and we consider that it will depend on the
preferences of the surgeon and the technique with which he is most comfortable, as well as
the patient's circumstances and his sports aspirations. No significant differences in outcome
have been found and there are no data to support that the percutaneous treatment is better
than the treatment by open surgery.
