Abstract

La mayoría de pacientes a los que se practica un trasplante de progenitores hematopoyéticos han recibido previamente múltiples transfusiones de hematíes. Cada transfusión recibida se traduce en 0.2 gramos de hierro que se depositan en el organismo sin que exista ningún sistema fisiológico capaz de eliminar el hierro sobrante. Es lógico pensar que tras múltiples transfusiones muchos de estos pacientes sufran una importante sobrecarga corporal de hierro. En otros pacientes, como los aquejados de hemocromatosis primaria y secundaria, sabemos que la sobrecarga de hierro es fuente de toxicidad. En el caso especial de los pacientes sometidos a trasplante de progenitores hematopoyéticos, la sobrecarga de hierro puede ser especialmente peligrosa durante las fases de aplasia y de reducción significativa de las defensas inmunitarias por las que pasan estos pacientes. En este trabajo se investiga si la sobrecarga férrica es un trastorno frecuente entre los pacientes que reciben un autotrasplante de progenitores hematopoyéticos, si dicha sobrecarga se asocia con morbimortalidad infecciosa, si existe algún germen en particular asociado a las infecciones en el contexto de sobrecarga férrica y si la sobrecarga férrica comporta una mayor mortalidad durante el trasplante. Además se investiga la evolución de algunos parámetros metabólicos del hierro durante el trasplante. <br/>El trabajo está basado en cuatro artículos. En el primero se demuestra que el valor de ferritina pre-trasplante y de saturación de transferrina en el dia -4 del trasplante constituyen factores pronósticos de la mortalidad global y de la mortalidad tóxica del trasplante. En el segundo artículo se demuestra que la sobrecarga hepática de hierro es muy frecuente entre los pacientes que mueren tras un trasplante hematopoyético. Además se demuestra que es frecuente la asociación entre dicha sobrecarga y la muerte por infección invasiva por el hongo aspergillus. En el tercer artículo. Se investiga si diversos parámetros asociados a sobrecarga de hierro pueden predecir la aparición de toxicidad e infección durante los primeros tres meses tras el trasplante (mucositis grave, bacteriemias y fiebre). Se demuestra que el nivel de ferritina pre trasplante se asocia con dichas variables. Por último, en el último artículo se demuestra que la elevada saturación de transferrina que se produce durante el acondicionamiento del trasplante es debida al stop de la eritropoyesis.<br/>En general, estos trabajos demuestran que existe una asociación entre sobrecarga férrica e incremento de morbilidad precoz y mortalidad durante el trasplante hematopoyético. Con posterioridad a los artículos recogidos en esta tesis, otros artículos han estudiado el mismo tema y han llegado a la misma conclusión. Sin embargo, el diseño de estos estudios no permite contestar si también existe una relación causa-efecto. Podría ser que aquellos pacientes con más hierro posiblemente han sido más transfundidos, y quizá esa mayor necesidad de transfusiones indica asimismo un peor pronóstico de la enfermedad. Pero las importantes relaciones del hierro con la generación de radicales libre y toxicidad nos explican que pueda comportarse como un agente tóxico. Por otra parte, su valor importantísimo para la virulencia de los gérmenes también nos explica su papel en el incremento de la infección.<br/>Esta tesis constituye un trabajo pionero en este tema. Previa a la publicación de los trabajos que conforman esta tesis no existía prácticamente literatura sobre el efecto de la sobrecarga férrica en el curso del trasplante. Hemos observado que tras su publicación se han multiplicado los trabajos internacionales sobre este tema. Por ello es posible que en unos años tengamos respuesta a muchas de las preguntas que en esta tesis se plantean.


Most patients who practiced a haematopoietic stem cell transplantation have previously received multiple transfusions of red blood cells. Each received transfusion translates into 0.2 grams of iron, that are deposited in the body without any physiological system capable of removing excess iron. It is logical to think that after multiple transfusions, many of these patients suffer a significant iron overload. In other patients, such as those suffering from primary and secondary hemochromatosis, we know that iron overload is a source of toxicity. In the special case of patients undergoing haematopoietic stem cell transplantation, iron overload can be especially dangerous during the aplasia phase, with a significant reduction in the immune system. This paper investigates whether the iron overload is a common disorder among patients receiving an hematopoietic transplant, if this iron overload is associated with infectious morbidity, whether a particular germ is associated with infections in the context of iron overload and if the iron overload involves an increased mortality during the transplant. It also investigates the evolution of some iron metabolic parameters during the transplant.<br/>The work is based on four articles. The first one shows that the value of pre-transplant ferritin and transferrin saturation in the day -4 of conditioning are prognostic factors for overall mortality toxic related mortality. The second article shows that the hepatic iron overload is very common among patients who die after an hematopoietic transplant. It also shows that there is often an association between the iron overload and death from invasive infection by the fungus aspergillus. In the third article, we investigate whether various parameters associated with iron overload may predict the occurrence of toxicity and infection during the first three months after transplantation (severe mucositis, bacteraemia and fever). We show that the level of pre-transplant ferritin is associated with these three variables. Finally, the last article demonstrates that the high transferrin saturation that occurs during conditioning for transplantation is due to the stop of erythropoiesis.<br/>In general, these studies show an association between iron overload and increased morbidity during early hematopoietic transplantation and mortality . Since the articles collected in this thesis, other articles have studied the same subject and have reached the same conclusions. However, the design of these studies can not answer if a cause-effect relationship exists. It could be that patients with more iron may have been more transfused, and perhaps the greater need for transfusions also indicates a worse prognosis of the disease. But the relationships of iron with the generation of free radical toxicity may explain how this element can function as a toxic agent. Moreover, its major value to the virulence of the germs also explains his role in the increase of the infection.<br/>This thesis is a pioneering work on this issue. Before the publication of works that comprise this thesis there was virtually no literature on the effect of iron overload in the course of transplantation. After their publication others international works appeared about this topic. It is therefore possible that in some years answers about questions raised in this thesis will appear.
