Efecto de la administración de triglicéridos de cadena media y ácidos grasos poliinsaturados ω-3 en la prevención y tratamiento de la fibrosis hepática en un modelo murino (ratones C57BL/6) de esteatohepatitis no alcohólica

Abstract

En la enfermedad por hígado graso no alcohólico (NAFLD), la alteración en el metabolismo de lípidos, estrés oxidativo y alteración en la producción de citocinas en un hígado graso causan inflamación y fibrosis hepáticas.<br/>La obesidad y diabetes se asocian a cambios histológicos desde esteatosis simple hasta necroinflamación y fibrosis (esteatohepatitis no alcohólica o NASH), una de las manifestaciones de NAFLD. La NASH es precursora de cirrosis. La prevalencia de NAFLD y NASH es de 20 y 2-3%, respectivamente. En la actualidad no se ha destacado la efectividad de un tratamiento en particular. Sin embargo, el conocimiento de su fisiopatología (resistencia a la insulina, seguida de una lipoperoxidación hepática con afectación del parénquima) sugiere el uso de hipoglucemiantes y antioxidantes, además de la pérdida de peso. <br/>La obesidad y resistencia a la insulina, mediada por TNF-&#945;, aumentan la liberación de ácidos grasos libres (FFA) de los adipocitos, aumentando su concentración en hígado. Los FFA inducen estrés oxidativo y favorecen la lipoperoxidación, estimulando la expresión de TNF- &#945; en hepatocitos y adipocitos e induciendo así la resistencia a la insulina y fibrogénesis. No existen modelos animales que definan los cambios moleculares y fisiológicos en la transición entre esteatosis y esteatohepatitis debido a la falta de un modelo que presente todas las características clave de la NAFLD. <br/>Los modelos de animales para NASH han identificado la deficiencia de leptina y estrés oxidativo como los principales factores patogénicos de la esteatosis y esteatohepatitis. Los modelos más ilustrativos de esteatohepatitis se fundamentan en la hipótesis del "hit" múltiple, que sugiere que el hígado graso es especialmente vulnerable a oxidantes y a desarrollar esteatohepatitis cuando factores agresores generan suficientes oxidantes como para causar muerte de hepatocitos e inflamación. Así, la esteatohepatitis incrementa la sensibilidad a otros factores agresores que inducen fibrosis hepática, promoviendo la evolución hacia cirrosis. El modelo inducido por dieta deficiente en colina y metionina (DCM) destaca la importancia del aumento en la actividad del CYP 2E1 en los hepatocitos y stress oxidativo en la patogénesis de la esteatosis y NASH. <br/>La sustitución de los ácidos grasos de cadena larga por triglicéridos de cadena media (MCT) mejora la hepatitis alcohólica histológicamente. Los ácidos grasos poliinsaturados (PUFA) &#969;-3, por otro lado, mejoran la resistencia a la insulina, aumentan la &#946;-oxidación y disminuyen la lipogénesis. Su combinación facilita la incorporación de PUFA a las membranas celulares de los órganos clave y, con ello, aumentan los beneficios de la administración de PUFA &#969;-3.<br/>El objetivo principal de esta tesis fue observar el efecto de los MCT y PUFA &#969;-3 en la prevención y tratamiento de la fibrosis hepática en un modelo murino (ratones C57/BL6 de NASH inducido por dieta DCM). Se estudiaron 46 ratones durante 6 semanas (fase de prevención, administrando DCM y MCT, PUFA o su combinación) y 46 ratones durante 10 semanas (fase de tratamiento: 6 semanas de inducción con dieta DCM, y 4 de tratamiento con MCT, PUFA o su combinación).<br/>El modelo replicó la mayoría de los hallazgos publicados pero no se constató un incremento en malondialdehído hepático. En cambio, disminuyó la leptina plasmática y aumentó la adiponectina, aún en inflamación. Sin embargo, se corroboró el papel antiinflamatorio de la adiponectina (fase de prevención) y el proinflamatorio de la leptina (fase de tratamiento). Otra aportación de esta investigación fue estudiar la variación en las concentraciones de carnitina y actividad de CPT-2.<br/>Los MCT y PUFA&#969;-3, no fueron profilácticos de esteatosis hepática, y los PUFA &#969;-3 produjeron inflamación (no fibrosis). Los MCT produjeron más inflamación que los PUFA &#969;-3 en el tratamiento, pero ambos afectaron negativamente el índice histológico hepático.


In non-alcoholic fatty liver disease (NAFLD), lipid metabolism alteration, oxidative stress and alteration on cytokine production lead to liver inflammation and fibrosis.<br/>Obesity and diabetes are associated to hystological changes ranging from simple steatosis to necroinflammation and fibrosis (non-alcoholic steatohepatitis, NASH), one of the manifestations of NAFLD. NASH precedes cirrhosis. The prevalence of NAFLD and NASH is of 20 and 2-3%, respectively. At the present time, there is no single, effective treatment. However, the knowledge of its physiopathology (insulin resistance, followed by hepatic lipid peroxidation affecting the parenchima) suggests the use of hypoglicemiants and antioxidants, along with weight loss. <br/>Obesity and insulin resistance mediated by TNF-&#945; increase the liberation of free fatty acids (FFA) from adipocytes, therefore increasing their concentration in the liver. FFA induce oxidative stress, followed by lipoperoxidation, which in turn stimulates TNF- &#945; expression in hepatocytes and adipocytes and induces insulin resistance and fibrogenesis. There are no animal models that define the molecular and physiological changes occurring during the transition of steatosis to steatohepatitis due to the lack of a model that presents every feature key for the development of NAFLD. <br/>NASH animal models have identified leptin and oxidative stress as the main pathogenic factors of steatosis and esteatohepatitis. The most illustrative models of steatohepatitis are based on the multiple "hit" hypothesis, which suggests that fatty liver is particularly vulnerable to oxidants and to develop steatohepatitis when aggressive factors generate enough oxidants as to cause hepatocyte death and inflammation. Steatohepatitis increases liver sensibility to other aggresive factors inducing liver fibrosis, therefore favouring the evolution to cirrhosis. The model induced by a choline and methionine-deficient diet (CMD) underscores the importance of the increased hepatocyte CYP 2E1 activity and oxidative stress on the pathogenesis of steatosis and NASH. <br/>The substitution of long-chain fatty acid for medium-chain triglycerides (MCT) improves the histology in alcoholic hepatitis. Omega-3 polyunsaturated fatty acids (PUFA &#969;-3), on the other hand, improve insulin resistance, increase &#946;-oxidation and decrease lipogenesis. Its combination favours PUFA incorporation into the cellular membranes of key organs, therefore increasing the benefits of PUFA &#969;-3 administration.<br/>The main objective of this thesis was to observe the effect of MCT and PUFA &#969;-3 on the prevention and treatment of liver fibrosis in a murine, NASH model (C57/BL6 mice) induced by CDM diet. Forty-six mice were studied during 6 weeks (prevention phase, administering CDM and MCT, PUFA or their combination) and 46 mice during 10 weeks (treatment phase: 6 weeks of induction with CDM diet, and 4 weeks of treatment with MCT, PUFA or their combination).<br/>The model replicates most of the published findings, but no increase on hepatic malondialdehyde was found. Instead, plasmatic leptin decreased and adiponectin increased, even when inflammation was present. However, the anti-inflammatory role of adiponectin (prevention phase) and proinflammatory role of leptin (treatment phase) were demonstrated. Another contribution of this research was the study of the variation on the carnitine concentrations and CPT-2 activity.<br/>MCT and PUFA &#969;-3 did not prevent liver steatosis, and PUFA &#969;-3 produced inflammation (no fibrosis). MCT caused more inflammation than PUFA &#969;-3 during the treatment phase, but both had a negative effect on the hepatic histological score.
