Abstract

El Melanoma uveal (MU) es un tumor del que sabemos poco en comparación a otras neoplasias. Pese a ello, los estudios realizados se han dirigido a buscar factores pronósticos, inicialmente a nivel histológico, y posteriormente a nivel citogenético y molecular, buscando qué vías de señalización están implicadas en su desarrollo y evolución, para tratar de identificar aquellos casos que van a desarrollar metástasis e intentar descubrir posibles dianas terapéuticas para poder ser utilizadas en el contexto de una terapia sistémica. Las vías de señalización de MAPK y PI3K-AKT-mTOR se ha observado que están constitutivamente activas en el MU y se han visto involucradas en su desarrollo a través de múltiples mecanismos (que son a su vez diferentes a los descritos en melanoma cutáneo). Ambas participan y convergen en la traducción de proteínas CAP dependiente. Por lo tanto, las proteínas implicadas en las rutas de señalización de ambas vías, así como las proteínas involucradas en la traducción proteica CAP dependiente también podrían encontrarse desreguladas en la patogénesis molecular del MU. Con el fin de conocer nuevos factores pronósticos y/o nuevas proteínas “principales” susceptibles a ser moduladas mediante fármacos para conseguir nuevos abordajes terapéuticos en el MU, desde un punto de vista pragmático que permita el uso de los resultados derivados de este trabajo en la práctica asistencial diaria, en esta tesis doctoral se han estudiado las vías de MAPK y PI3K-AKT-mTOR, desde sus proteínas receptoras, centrándonos en HER3, hasta sus proteínas efectoras a nivel de control de la traducción como son EIF-4E y EIF-4G. Primero se ha realizado un estudio basado en la expresión inmunohistoquímica de los marcadores que forman parte del “signaling”. Los resultados se han relacionado con los factores pronósticos clásicos descritos en la literatura y valorados en la serie de casos de nuestra muestra, así como con el seguimiento y evolución de los pacientes incluidos en la misma. Además se han realizado estudios in vitro para valorar la respuesta de tres líneas celulares de melanoma uveal a Dacarbacina y otras 2 drogas que sabemos son inhibidoras de cada una de las vías descritas (PP242 como inhibidor de la vía de PI3K-AKT-mTOR y AZD 8330 como inhibidor de la vía de MAPK), comparando el efecto sobre la viabilidad celular y la capacidad de formación de colonias de cada uno de los tres fármacos así como su combinación. La doctoranda ha podido demostrar como la expresión de HER3 actúa como variable independiente predictora del riesgo de aparición de metástasis, teniendo peor pronóstico los niveles de expresión inmunohistoquímica bajos, y que la expresión de P16 y Ki67 actúan como variable independiente de supervivencia global teniendo más riesgo de muerte aquellos pacientes con expresión de P16 baja-moderada y Ki67 elevado. De manera interesante ha podido demostrar como el uso combinado de dichos marcadores predice el riesgo de muerte de manera mas precisa. Por otro lado, en el presente trabajo se ha podido observar como los factores pronósticos clínicos como edad, estadiaje al diagnóstico y localización del tumor junto con los histológicos como mitosis, histología e infiltrado inflamatorio, también actúan como variables pronósticas independientes de riesgo de metástasis, riesgo de muerte por enfermedad y supervivencia global en nuestra serie. Finalmente en los estudios in vitro se ha podido observar como el uso combinado de cada uno de los inhibidores combinados con el agente quimioterápico (dacarbacina) ha mostrado tener un efecto sinérgico sobre la viabilidad celular que abre una nueva ventana de oportunidad terapéutica que deberá ser explorada con mayor detalle en posteriores estudios de investigación.


The uveal melanoma (UM) is a tumor of which we know little in comparison to other neoplasias. In spite of this, the studies carried out have aimed to look for prognostic factors, initially at the histological level, and later at the cytogenetic and molecular levels, looking for which signaling pathways are involved in its development and evolution, in order to try to identify those cases that go to develop metastases and try to discover possible therapeutic targets to be used in the context of a systemic therapy. The signaling pathways of MAPK and PI3K-AKT-mTOR have been found to be constitutively active in UM and have been involved in its development through multiple mechanisms (which are in turn different from those described in cutaneous melanoma). Both participate and converge in the translation of CAP dependent proteins. Therefore, the proteins involved in the signaling pathways of both pathways as well as the proteins involved in the CAP dependent protein translation could also be found deregulated in the molecular pathogenesis of UM. In order to know new prognostic factors and / or new "main" proteins susceptible to be modulated by drugs to obtain new therapeutic approaches in the UM, from a practical point of view that allows the use of the results derived from this doctoral thesis, the pathways of MAPK and PI3K-AKT-mTOR have been studied, from their receptor proteins, focusing on HER3, to their effector proteins at the level of translation control, such as EIF-4E and EIF- 4G. First, a study was carried out based on the immunohistochemical expression of the markers that are part of the signaling. The results were related to the classic prognostic factors described in the literature and evaluated in the case series of our sample, as well as to the follow-up and evolution of the patients included in the sample. In addition, in vitro studies have been conducted to evaluate the response of three cell lines of uveal melanoma to Dacarbacine and two other drugs known to be inhibitory to each of the pathways described (PP242 as an inhibitor of PI3K-AKT-mTOR and AZD 8330 as an inhibitor of the MAPK pathway), comparing the effect on cell viability and the colony forming ability of each of the three drugs as well as their combination. The doctoral student has been able to demonstrate how the expression of HER3 acts as an independent predictor of the risk of metastasis, with poor prognosis at low levels of immunohistochemical expression of HER3, and that the expression of P16 and Ki67 act as an independent variable of global survival taking more risk of those patients with low-moderate of P16 expresion and high-Ki67 expression. Interestingly, she has been able to demonstrate how the combined use of such markers predicts the risk of death more accurately. On the other hand, in the present work we have observed how the clinical prognostic factors such as age, staging at the diagnosis and location of the tumor together with the histological ones such as mitosis, histology and inflammatory infiltrate also act as independent prognostic variables of risk of metastasis, risk of death by disease and overall survival in our series. Finally in the in vitro studies it has been observed how the combined use of each of the inhibitors combined with the chemotherapeutic agent (dacarbacine) has been shown to have a synergistic effect on the cellular viability that opens a new window of therapeutic opportunity that should be explored in greater detail in subsequent research studies.
