Abstract

Introducció: Una prioritat formativa en emergències per als professionals d’atenció primària és el suport vital immediat (SVI) o atenció immediata al pacient crític, i que inclou tot el relacionat amb el suport vital bàsic (SVB) i l’ús del desfibril·lador automàtic (DEA). El curs de SVI està basat en la metodologia A(Airway) – B(Breathing) – C(Circulation) – D(Disability) – E(Exposure), i tot pacient crític hauria de ser tractat segons aquesta metodologia i amb un estricte ordre. En el nostre entorn existeixen dos protocols d’actuació urgent en infart agut de miocardi (Codi IAM) i ictus (Codi Ictus). La formació basada en simulacions és una eina d’ús creixent aquests últims 10 anys. Objectius: Elaborar un instrument per mesurar habilitats en SVI tipus checklist i estandarditzar un model de dinamització i de formació. Elaborar escenaris de simulació d’atenció inicial al pacient crític en un entorn d’atenció primària. Avaluar l’impacte d’una activitat formativa entre metges i infermeres d’atenció primària en el nivell de coneixements i habilitats a curt i mig termini. Avaluar la validesa i acceptabilitat d’un sistema de formació en SVI basat en activitats pràctiques simulades amb feedback immediat. Tipus d’estudi: Estudi d’intervenció, sense grup control, de caràcter obert i de tipus abans-després. Material i mètodes: Checklist del Codis IAM i Ictus per registre i dinamització de la simulació. Escenaris elaborats (cas clínic predefinit). Qüestionari de valoració qualitativa de les simulacions per dinamitzadors i rescatadors/assistents. Test de coneixements de 5 preguntes test. Primera sèrie de simulacions als 8 centres d’atenció primària participants, i als 3 i 6 mesos segona sèrie amb els mateixos centres, dinamitzadors i casos clínics. Es fan avaluacions in vivo d’aquestes simulacions, a una i tres setmanes per valorar concordances i fiabilitats. Resultats: Es van realitzar 48 simulacres vídeo gravats, participant 16 dinamitzadors, 16 rescatadors, 240 alumnes, utilitzant 8 maniquins/pacients simulats (voluntaris). Es van respondre 449 tests de coneixements, que van demostrar puntuacions superiors al final (3,89/5 - DE 1,01) que a l’inici (3,21/5 - DE 1,09), millors puntuacions en professionals que ja han finalitzat la seva formació tant a l’inici (3,27/5 - DE 1,09) com al final (3,89/5 - DE 0,99), i millors puntuacions en metges (3,81/5 - DE 0,97) que en infermeres (3,32/5 - DE 1,15), totes estadísticament significatives. La mitjana de puntuacions de la primera sèrie de simulacres sobre un màxim de 10 punts va ser de 7,7 punts (DE 15,6%), en la segona sèrie de 8,2 (DE 10,6%) en l’avaluació in vivo, de 8,7 (DE 14,8%) en l’avaluació diferida i de 9,1 (DE 7,8) en l’avaluació a les 3 setmanes. Les concordances en les avaluacions de la segona sèrie de simulacions comparant les avaluacions in vivo amb les diferides i amb les fetes a les 3 setmanes, i estratificant els resultats per a cadascuna de les 7 etapes, s’obté un índex Kappa superior a 0,70 (fiabilitat excel·lent) en 2 de les 7 etapes i un índex Kappa d’entre 0,4 i 0,69 (fiabilitat bona) en 5 de les 7 etapes. Conclusions: S’ha elaborat un instrument per mesurar habilitats en SVI tipus checklist i s’ha estandarditzat un model de dinamització i formació. S’han elaborat escenaris de simulació d’atenció inicial al pacient crític amb els codis IAM i Ictus, en atenció primària. L’impacte de l’activitat formativa en SVI entre metges i infermeres millora el nivell de coneixements. S’observa una tendència creixent en les mitjanes de puntuacions dels checklists a mesura que progressen els simulacres, el que es tradueix en una millora de les habilitats a curt i mig termini. Un sistema de formació en SVI basat en activitats pràctiques simulades amb feedback immediat es considera vàlid, fiable i acceptable.


Primary Care Introduction: Immediate life support (ILS) or immediate attention to critical patients are a priority emergency training for primary care professionals. ILS includes everything related to basic life support (BLS) and the use of the defibrillator. The course of ILS is based on the methodology A (Airway) - B (Breathing) - C (Circulation) - D (Disability) - E (Exposure), and all critically ill patient should be treated according to this methodology and a strict order. In our environment there are two urgent protocols to treat myocardial infarction (Codi IAM) and stroke (Codi Ictus). The simulation-based training is a tool with increasing use these last 10 years. Objectives: Develop an instrument to measure skills checklist type ILS and standardize a model of dinamization and training. Develop scenarios simulating initial attention to critical patients in a primary care setting. Evaluate the impact of training activities between primary care doctors and nurses in the level of knowledge and skills in the short and medium term. Assess the validity and acceptability of a system of training in simulated ILS based on activities with immediate feedback. Type of study: intervention group without control, open and before-after type. Methods: Checklist of “Codi IAM” and “Codi Ictus” by recording and dynamic simulation. Scenarios developed (predefined clinical case). Questionnaire qualitative assessment of simulations and rescuers facilitators / assistants. Knowledge test of five quiz questions. First series of simulations in 8 health center of primary care participants at 3 and 6 months and second series with the same health centers, promoters and clinical cases. In vivo evaluations are made of these simulations, one and three weeks to evaluate concordances and reliabilities. Results: We performed 48 simulations video recorded with 16 facilitators, 16 rescuers, 240 students and 8 mannequins/simulated patients (voluntary). They replied 449 knowledge tests, which showed higher scores in the final (3.89 / 5 – SD 1.01) than at initials (3.21 / 5 – SD 1.09), better scores on professionals that had already completed their medical speciality both at the beginning (3.27 / 5 - SD 1.09) and the end (3.89 / 5 – SD 0.99), and higher scores on doctors (3.81 / 5 - 0.97) than nurses (3.32 / 5 – SD 1.15), all statistically significant. The average scores of the first series of simulation based training on a maximum of 10 points was 7.7 points (SD 15.6%), in the second series was 8.2 points (SD 10.6%) in the evaluation in vivo, 8.7 points (SD 14.8%) in assessing deferred and 9.1 points (SD 7.8) in the assessment at 3 weeks. The matches in the evaluations of the second series of simulations compared with in vivo evaluations with deferred and taken to three weeks, stratifying the results for each of the seven stages, was a higher Kappa index 0.70 (excellent reliability) in two of the seven stages and a Kappa index of between 0.4 and 0.69 (good reliability) in five of the seven stages. Conclusions: We developed a tool to measure skills and standardized checklist type ILS model of dinamitation and training. Simulation scenarios have been developed for initial care to critical patients with myocardial infarction and stroke in primary care. The impact of the training activity ILS between doctors and nurses improve the level of knowledge. There is a growing trend in the average scores of checklists as their progress of the simaltion based training, which results in an improvement of skills in the short and medium term. ILS training system based on activities with immediate feedback simulated is considered valid, reliable and acceptable.
