Abstract

Las alteraciones tiroideas se han propuesto como participantes importantes en la patogénesis de las psicosis afectivas y no afectivas. Tales alteraciones pueden modular la expresión de la enfermedad con implicaciones sobre su gravedad, evolución y pronóstico. Estudios previos han mostrado la influencia de la función y autoinmunidad tiroideas en la evolución de la enfermedad en pacientes psicóticos y bipolares. Sin embargo, el mecanismo de acción de esta modulación no ha sido aún esclarecida. Los estudios que investigan la relación entre los niveles de hormonas tiroideas y el estado de la autoinmunidad tiroidea y las funciones cognitivas, han encontrado resultados inconsistentes y contradictorios. Sabemos que las alteraciones cognitivas tiene un gran impacto sobre la funcionalidad de los pacientes psicóticos y el pronóstico global de la enfermedad. El objetivo principal de este trabajo es esclarecer el papel real que juega la función y la autoinmunidad tiroideas en la modulación de la evolución de los trastornos psicóticos afectivos y no afectivos, en 3 aspectos; gravedad de síntomas, riesgo de recaídas y afectación cognitiva. Así mismo, estudiamos si esta modulación sobre la afectación cognitiva es diferente en pacientes con psicosis afectivas y no afectivas. Para ello estudiamos tres muestras de pacientes. Una muestra de pacientes con trastorno bipolar (N=77) que han requerido hospitalización por un episodio maníaco o mixto, seguidos durante un año para estudiar el riesgo de reingreso. Para el estudio de la relación entre función y autoinmunidad tiroideas y rendimiento cognitivo se han utilizado dos muestras diferentes: una muestra de 70 pacientes con una psicosis temprana (estudio transversal que incluye la diferenciación de los efectos sobre cognición en psicosis afectivas y no afectivas) y una muestra de 36 pacientes con un primer episodio psicótico (estudio longitudinal de un año de evolución). De nuestros resultados podemos concluir que la función y autoinmunidad tiroideas no se asocian con la gravedad de síntomas maniacos o psicóticos en paciente con trastorno bipolar o psicosis tempranas. Los anticuerpos antitiroglobulina (pero no antiperoxidasa) se asocian con un menor riesgo de rehospitalización en pacientes bipolares después de un ingreso por episodio maniaco o mixto. Por otro lado, niveles más altos de tiroxina libre (T4L), aunque dentro de rango de normalidad, se asocian con un mejor rendimiento en la atención y vigilancia en pacientes afectos de una psicosis temprana, sin embargo, la autoinmunidad tiroidea no parece afectar de la misma manera al rendimiento cognitivo. Los niveles de T4L se asocian a cambios longitudinales en el rendimiento en atención y vigilancia al año, niveles extremos de T4L (aunque dentro de la normalidad), se asocian con un empeoramiento en este dominio cognitivo. La asociación entre T4L y habilidades cognitivas es más evidente en los paciente afectos de psicosis afectivas (trastorno bipolar o trastorno esquizoafectivo) en el dominio de atención y vigilancia.


Thyroid abnormalities have been proposed to be important elements in the pathogenesis of affective and non-affective psychoses. Such alterations may modulate the expression of the illness with implications on its severity and prognosis. Previous studies show the influence of the function and thyroid autoimmunity on the evolution of the disease in bipolar and psychotic patients. However, the mechanism of action of this modulation has not been clarified yet. Studies searching the relationship between thyroid hormones levels and thyroid autoimmunity and cognitive functions have found contradictory and inconsistent results. Cognitive impairment has a major impact on the functionality of psychotic patients and the overall prognosis of the disease. We aimed to clarify the role of thyroid function and autoimmunity on the modulation of the evolution of affective and non-affective psychotic disorders in three aspects; severity of symptoms, risk of relapse and cognitive impairment. We also studied if this modulation is different in affective and non-affective psychoses. For this purpose we studied three patient samples. One sample of bipolar patients (N=77) requiring hospitalization for a manic or mixed episode that were followed up for one year to study the risk of readmission. For the study of the relationship between thyroid function and autoimmunity and cognitive abilities, two different samples were used: one sample of 70 patients with an early psychosis (cross-sectional study including the differentiation of the effects on cognition in affective and non-affective psychoses) and a sample of 36 patients with a first psychotic episode (one year longitudinal study). From our results, we can conclude that thyroid function and autoimmunity are not associated with the severity of manic or psychotic symptoms in patients with bipolar disorder or early psychosis. Thyroglobulin antibodies (but not peroxidase antibodies) are associated with a lower risk of rehospitalization of bipolar patients after a manic or mixed episode. On the other hand, higher levels of free thyroxine (FT4), but within normal range, are associated with better performance on attention and vigilance domain in patients with an early psychosis. However, thyroid autoimmunity does not seem to affect cognitive performance in the same way. FT4 levels are associated with longitudinal changes in the performance in attention and vigilance at one year. Extreme levels of FT4 (although within normal levels) are associated with a decline in this cognitive domain. The association between FT4 and cognitive skills is most evident in patients with an affective psychosis (bipolar disorder or schizoaffective disorder) in attention and vigilance domain.
