Monitorización continua y no invasiva de la hemoglobina intraoperatoria: ¿Exactitud o información a tiempo real?

Abstract

La anemia y el sangrado son complicaciones frecuentes durante la cirugía mayor oncológica. La determinación de la hemoglobina es una de las mediciones de laboratorio más frecuentes en el transcurso de la cirugía para guiar la terapia transfusional. Varios estudios han mostrado una asociación entre morbi-mortalidad y transfusión en los pacientes quirúrgicos oncológicos, por lo que se necesitan aparatos de laboratorio que proporcionen medidas rápidas y exactas para reducir las transfusiones innecesarias. Los analizadores GEM® PremierTM 4000 y ABL 800 FLEX son del tipo point-of-care-testing (POCT), o de cabecera del paciente. Están ubicados en el área quirúrgica, precisan una muestra de 2 ml de sangre y ofrecen los resultados en 2 minutos. El sistema Radical_7, comercializado desde el año 2007, ofrece la ventaja de medir la hemoglobina de forma continua y sin necesidad de extraer sangre, a través de un pulsioxímetro colocado en el dedo. Los objetivos del estudio fueron: (1) conocer la exactitud y precisión del Radical_7 y de los analizadores POCT con respecto al sistema de referencia; (2) conocer qué variables afectan al sistema de medición del Radical_7. Para la comparación se obtuvieron muestras sanguíneas de 115 pacientes de cirugía hepática, pancreática y biliar que se midieron simultáneamente en los diferentes analizadores. Los resultados mostraron que el analizador ABL 800 presentó la mejor exactitud y correlación con respecto al sistema de referencia (sesgo = -0,35 g/dl, IA 95%: -1,27 a 0,56 g/dl), y sería el más indicado para guiar la transfusión intraoperatoria. En relación a la exactitud del Radical_7, ninguno de los factores individuales estudiados influyó de forma significativa. Sólo la temperatura fue capaz de explicar un índice de perfusión óptimo del dedo, aunque con un valor predictivo muy bajo (ROC = 0,58). No descartamos la existencia de otros factores no estudiados capaces de influir en la exactitud del aparato.


Anaemia and bleeding are frequent complications during major surgery. Haemoglobin concentration is one of the most common laboratory analysis performed daily in order to guide transfusion therapy. Several studies have demonstrated a strong relationship between transfusion and an increase in morbidity and mortality in surgical patients. Thus we need rapid and precise laboratory analysers in order to reduce unnecessary transfusions. Point-of-care testing (POCT) analysers such as the GEM®PremierTM4000 (Laboratory Instrumentation) and the ABL 800 FLEX (Radiometer, Denmark) can be found in surgical areas. They require a blood sample of 2 ml and 2 minutes only in order to produce the results. Commercialized since 2007, the Radical_7 monitor (Masimo Corp, CA), is a device for measuring haemoglobin in real-time, continuously and non-invasively by pulse oximetry. The goals of the study were: (1) to know the accuracy and precision of the POCT and Radical_7 analysers as compared to the reference method: the Sysmex XE-5000 (Roche Diagnostic, SL); (2) to learn which individual variables affect the measuring system of Radical_7. One hundred and fifteen patients who underwent hepatic or pancreatic surgery were included and 321 blood samples were measured simultaneously in the different analysers. Our results showed that the ABL 800 FLEX analyser presented the best accuracy and correlation as compared to the reference method (bias = 0.35 g/dl, IA 95%: 1.27 – 0.56 g/dl) and would be of choice to guide intraoperative transfusion therapy. Regarding the accuracy of Radical_7, any of the individual factors studied proved having a strong influence. Only central temperature was related to an optimum perfusion index of the finger where the sensor is located, but with a very low predictive value (ROC = 0,58). We do not reject the existence of others unknown factors which might influence the Radical_7 accuracy.
