Abstract

La inclusió dels familiars com agents importants en el tractament de les persones amb trastorns mentals greus (TMG) és beneficiosa per a la millora de l’estat clínic dels malalts. La finalitat d’aquesta investigació era estudiar l’adaptació dels cuidadors no professionals de persones amb TMG, des del marc teòric d’estrès-apreciació-afrontament. S’han avaluat les fonts d’estrès (les pròpies característiques del TMG i l’apreciació de la situació), les respostes d’afrontament, alguns factors moduladors personals i socials, i les conseqüències com ara el benestar i la qualitat de vida relacionada amb la salut dels cuidadors (QV). OBJECTIUS- Adaptar a la nostra població una mesura d’apreciació de l’estrès que inclou aspectes negatius i positius de la situació de tenir cura de persones amb TMG, el qüestionari “Experience of Caregiving Inventory, ECI” (ECI, Szmukler et al, 1996), creant la versió en castellà (Estudi 1). Descriure el perfil del cuidador dels pacients atesos a un dels centres assistencials de referència de la regió sanitària Catalunya Central i estudiar els factors explicatius de la percepció de la situació, de l’emoció expressada (EE) i de la QV (Estudi 2). SUBJECTES- Estudi 1) Inclou dues mostres de cuidadors: una dels malalts mentals atesos a la Fundació Althaia de Manresa i l’altra dels associats de la Federació Salut Mental Catalunya. Estudi 2) Cuidadors de malalts amb TMG atesos a la Fundació Althaia de Manresa. INSTRUMENTS- Protocol que inclou consentiments informats, dades sociodemogràfiques i dades clíniques, de pacients i cuidadors, i instruments estandaritzats (EEAG, ECI, COPE abreujat, escala de competència personal percebuda, Family Questionnaire, EVA de suport social percebut, SF-36, SCL-90-R, escala de sobrecàrrega Zarit,). PROCEDIMENT- Disseny metodològic prospectiu, transversal i amb seguiment als 6 mesos. RESULTATS- En la validació de la versió en castellà de l’ECI, l’anàlisi ha confirmat dues escales d’aspectes negatius relacionades amb símptomes nuclears dels TMG (conductes disruptives i símptomes negatius) i dues d’aspectes positius, i es pot considerar un instrument d’autoinforme vàlid, fiable, breu, comprensible i factible per a la seva administració en el context sanitari i comunitari. El perfil dels cuidadors no difereix significativament del descrit en estudis similars. La percepció positiva de la situació per part dels cuidadors pot explicar-se en part per l’estil d’afrontament “eficaç” (actiu o de compromís). La gravetat del trastorn mental no és suficient per a explicar l’EE i la QV, sinó que l’EE s’explica millor per la percepció negativa de la situació dels cuidadors, i nivells més alts d’EE s’associen amb més ús d’estratègies d’afrontament “menys eficaces” (evitadores i de cerca de suport) i també amb menor percepció de control dels cuidadors. L’estil d’afrontament evitador i el criticisme es relacionen negativament amb la QV. Els principals factors predictors de millor QV són la menor sobreimplicació emocional i la major percepció de control dels cuidadors. En l’explicació de la variabilitat del component físic de la QV, a més intervenen significativament algunes característiques dels cuidadors (menor edat, sexe masculí, passar més temps amb el malalt). En absència de canvis en l’estatus de la malaltia mental, la qualitat de vida dels cuidadors es manté estable en el seguiment als 6 mesos. La QV relacionada amb la salut mental dels cuidadors de malalts hospitalitzats millora amb el pas dels temps, quan en la majoria dels casos els pacients han passat al dispositiu de tractament ambulatori. Quan el pacient es troba en recurs ambulatori, el seu millor funcionament global prediu un balanç més positiu que negatiu en la percepció de la situació del cuidador. La identificació d’aquests factors significatius per a l’adaptació i el benestar dels cuidadors pot ser útil en l’estudi de noves propostes d’intervencions familiars en TMG.


The inclusion of family members as important agents in the treatment of people with severe mental disorders (SMD) is beneficial to the improvement of the clinical status of the patients. The purpose of this research was to study the adaptation of non professional caregivers of people with SMD, within the framework of stress-appraisal-coping. Sources of stress (the characteristics of SMD and appraisal of the situation), coping responses, some factors modulating personal and social consequences such as wellbeing and the quality of life related to health caregivers (QOL) were evaluated. OBJECTIVES- To adapt to our population a measure of appreciation of stress that includes negative and positive aspects of the situation of caring for people with SMD, the questionnaire “Experience of Caregiving Inventory, ECI” (ECI, Szmukler et al, 1996), creating the Spanish version (Study 1). To describe the profile of caregivers of patients treated at one of the reference medical care centers in the health region of Catalunya Central and to study the explaining factors of the appraisal of the caregivers, their expressed emotion (EE) and their QOL (Study 2). SUBJECTS- Study 1) This includes two samples of caregivers: one sample of patients with mental disorders attended the Fundació Althaia de Manresa and another one of the members of the Federació Salut Mental Catalunya. Study 2) Caregivers of SMD patients attended the Fundació Althaia de Manresa. MATERIALS- Protocol that includes informed consents, demographic data, and clinical data of patients and caregivers, and standarized measures (EEAG, ECI, COPE brief, perceived personal competence scale, Family Questionnaire, VAS of perceived social support, SF-36, SCL-90-R, Zarit burden scale). PROCEDURE- Prospective methodological design, transversal and follow up 6 months. RESULTS- In the validation of the Spanish version of the ECI , analysis has confirmed two scales of the negative aspects associated with core symptoms of the SMD (disruptive behaviors and negative symptoms) and two positive aspects, and it can be considered a valid, reliable, brief , understandable and feasible self-report measure for its administration in the health and community context. The profile of caregivers did not differ significantly from those described in similar studies. The positive appraisal of the caregivers can be explained in part by “effective” coping style (active or involved). The severity of the mental disorder is not enough to explain the EE and the QOL, but EE is best explained by the negative appraisal of caregivers, and higher levels of EE are associated with greater use of “less effective” coping strategies (evasive and support searching) and with less perceived control of the caregivers. The evasive coping style and criticism are negatively associated with QOL. The main predictors of best QOL are the lower level of overinvolvement and the greater perceived control of the carers. In the explanation of the variability of the physical component of QOL some significant characteristics of caregivers are also involved (younger, male, spends more time with the patient). In the abscence of changes in the status of the mental disorder, the QOL of the caregivers remains stable during the 6 months follow-up. The QOL related to the mental health of the caregivers of inpatients improves as time passes, when in the majority of cases the patients have passed to outpatient treatment. When the patient is in outpatient treatment, his improved global functioning predicts a balance that is more positive than negative in the appraisal of the situation of the caregiver. The identification of this factors that are significative for the adaptation and the wellbeing of the caregivers can be useful in the study of new proposals for family interventions in SMD .
