Bases para la adquisición de una cultura preventiva ante los riesgos biológicos en los estudiantes de Grado de Enfermería de la Universidad de Barcelona

Resumen

OBJETIVOS: analizar el impacto que tienen las prácticas clínicas sobre el nivel de conocimientos de la Prevención de Riesgo (PRB) y sobre la percepción del Riesgo Biológico (RB) en los estudiantes de Grado de Enfermería y explorar la relación entre el nivel de conocimientos sobre la PRB, la percepción del RB y la utilización de las medidas preventivas utilizadas por los estudiantes durante dichas prácticas. METODOLOGÍA: estudio cuasi experimental, con mediciones pretest y postest, realizado en la Escuela Universitaria de Enfermería de la Universidad de Barcelona. La muestra fue de 227 estudiantes matriculados en la asignatura práctica de Estancias Clínicas II (ECII) durante el curso 2013-2014. Variables: características sociodemográficas, laborales, formativas y estado de vacunación; nivel de conocimientos sobre PRB; evaluación dimensional del RB percibido; utilización de las medidas de prevención frente al RB; accidentes sufridos. Instrumento ad hoc: Cuestionario para el estudio de la Prevención del Riesgo Biológico en los estudiantes de Grado de Enfermería (CuPRBGE). RESULTADOS: la media de edad fue de 24 años (DE 5.8) y el 79.7% (n=181) eran mujeres. Vacunación: el 90.7% (n=206) de los estudiantes declararon estar vacunados contra la hepatitis A, el 96.9% (n=220) contra la hepatitis B, el 96.0% (n=218) contra el tétanos y el 21,1% (n=48) contra la gripe. En relación a la prueba de la tuberculina, el 23.4% no se la había hecho o bien no lo recordaba (n=53). Respecto al nivel de conocimientos, los estudiantes presentaron una media total de 29.2 pretest y de 29.7 postest. Respecto al RB percibido, se encontraron diferencias significativas en el conocimiento del RB percibido por los estudiantes (p=0.005) y en la percepción de la inmediatez de las consecuencias (p=0.055) en el sentido de que los estudiantes después de realizar las prácticas presentaron mayor percepción del RB. El 25.1% de los estudiantes refirieron haber tenido un accidente, de los cuales el 19,3% no fueron declarados. Al relacionar los conocimientos, la percepción y la utilización de los EPIs, se encontró una relación significativa en el sentido de que los estudiantes con una alta percepción global del RB y que presentaban niveles más altos de conocimientos sobre los EPIs, utilizaban mejor dichos EPIs. CONCLUSIONES: el nivel de conocimientos global sobre la PRB, el grado de percepción respecto al conocimiento del RB y respecto a la inmediatez de las consecuencias de los accidentes, fue mayor postest respecto al pretest. En los estudiantes con una alta percepción del RB existe una asociación significativa entre el nivel de conocimientos y la utilización de dichos EPIs.


OBJECTIVES The general objectives of this study were two: to explore the relationship between the level of knowledge about biohazard prevention, the perception of biological risk and the use of preventive measures used by nursing degree students; to analyse the impact of these clinical training on knowledge of the biohazard prevention and on the perception of biological risk in the students METHODOLOGY The quasi-experimental study at the University School of Nursing of the University of Barcelona. The sample consisted of 227 students enrolled in the subject of Clinical Stage II (CSII) for the 2013-2014 year. Variables: socio-demographic, employment, training and vaccination; knowledge of biohazard prevention; dimensional evaluation of biological risk perceived; use of preventive measures against biohazard; suffered through accidents. Instrument: Questionnaire for the study of biohazard prevention in nursing degree students. For the descriptive analysis of the categorical variables, frequencies and percentages and quantitative variables, the mean and Standard Deviation (SD) or median and range are calculated. For the bi-variation analysis, the Student Fisher t test or ANOVA for categorical variables and the Pearson correlation coefficient for quantitative variables is used. RESULTS Of the 227 students enrolled in the study, the mean age was 24 years (SD 5.8) and 79.7% (n = 181) were female. 71.8% (n = 163) had not worked and 17.6% (n = 40) had taken courses regarding risk prevention. Vaccination: 90.7% (n = 206) of students report being vaccinated against hepatitis A, 96.9% (n = 220) against hepatitis B, 96.0% (n = 218) against tetanus and 21.1 % (n = 48) flu. In relation to the tuberculin test, 23.4% had not done it or did not remember (n = 53). Regarding the level of knowledge, the students presented a total average of 29.2 before starting the CSII and 29.7 thereafter, presenting the lowest scores in the categories: general standard precautions and post-exposure protocols. Regarding the biological risk perceived, significant differences in the perceived knowledge RB (p = 0.005) and in the perception of the immediate consequences (p = 0.055) were found to be higher after the CSII baseline. 25.1% of students reported having had an accident, of which 19.3% were undeclared. By relating the knowledge, perception and the use of Individual Protection Equipment (IPE), a significant relationship was found in the sense that, students with a high overall perception of biological risk and who had higher levels of IPE knowledge, had best used said IPE. CONCLUSIONS: The overall level of knowledge about biohazard prevention that students present at the end of the CSII was higher compared to the beginning of such practices. The degree of perception of students regarding knowledge of the biohazard and the immediacy of the consequences of the accidents, was greater after the baseline. For students with a high perception of biological risk there is a significant association between the level of knowledge and the use of such IPE.
