Clonalidad hematopoyética en la policitemia vera y la trombocitemia esencial y su papel en la transformación mielofibrótica

Abstract

Las neoplasias mieloproliferativas (NM) son enfermedades clonales originadas en una célula madre hematopoyética caracterizadas por la proliferación de células maduras de una o varias líneas mieloides y que presentan cierto riesgo de progresión a leucemia aguda o a presentar fallo medular debido a la aparición de mielofibrosis, durante la evolución de la enfermedad. En el año 2005 se describió la mutación JAK2V617F que se detecta en el 95% de la Policitemia Vera (PV) y en el 60% de la Trombocitemia Esencial (TE) y la Mielofibrosis Primaria (MFP). Posteriormente, se describieron otros marcadores moleculares implicados en vías de señalización (LNK, SOCS, CBL), en procesos epigenéticos (TET2, ASXL1, IDH1, IDH2, EZH2) o en la maquinaria del splicing del ARN mensajero (SF3B1, SRSF2, U2AF1). En general, estos marcadores se observan en bajos porcentajes en las NM y ninguno es específico de estas enfermedades. Más recientemente, a finales del año 2013, se reportó la presencia de mutaciones en el gen CALR en un 49-84% de las TE y un 67-88% de las MFP JAK2V617F negativas, convirtiéndose en el segundo marcador molecular más frecuente de las NM. Desde su descubrimiento, la mutación JAK2V617F ha sido un criterio para el diagnóstico de las NM clásicas (PV, TE, MFP). Dada su elevada frecuencia, las mutaciones en CALR ya se han propuesto como futuro criterio diagnóstico a incluir en las próximas guías de clasificación de la OMS, mientras que el papel a nivel diagnóstico de otros marcadores es más limitado. Sin embargo, existen varios estudios que reportan que la presencia de alteraciones adicionales a JAK2 o CALR pueden tener un papel en la progresión de la TE o la PV a mielofibrosis o leucemia aguda mieloide. El objetivo principal del trabajo fue identificar si la detección de una hematopoyesis clonal puede tener un papel en la transformación de las NM. Los dos primeros trabajos se basaron en el estudio de una cohorte de pacientes, principalmente con TE JAK2V617F negativos. Lo resultados mostraron que la presencia de alteraciones en nuevos marcadores moleculares como TET2, ASXL1, IDH1/2 y c-CBL son infrecuentes en este grupo de pacientes. También se observó que en la TE JAK2V617F negativa, la detección de clonalidad hematopoyética mediante el análisis del patrón de inactivación del cromosoma X, analizando el polimorfismo del gen HUMARA, se asocia a mayor riesgo de transformación mielofibrótica. Además, aunque los resultados no fueron estadísticamente significativos, se observó cierta tendencia a la asociación entre presentar alteraciones distintas a JAK2V617F y la transformación de la enfermedad. En el tercer trabajo, se realizó un estudio mutacional de nuevos marcadores, en una cohorte de pacientes con mielofibrosis post-trombocitémica (MF post-TE) y mielofibrosis post-policitémica (MF post-PV) y se comparó con el de los pacientes con NM en fase crónica. Los resultados mostraron que la presencia de mutaciones en los genes del spliceosoma son infrecuentes en la TE y PV en fase crónica, y que existe un perfil de mutaciones de los genes de la maquinaria del splicing distinto entre los pacientes con MFP y los pacientes con MF post-TE o MF post-PV. De forma similar, en el grupo de TE y PV con mielofibrosis secundaria, no se pudo demostrar significación estadística entre la presencia de mutaciones adicionales a JAK2 y el riesgo de transformación mielofibrótica. Aun así, cabe destacar que la incidencia de mutaciones era superior en los pacientes evolucionados respecto a los de fase crónica, acorde también a lo descrito en la literatura.


Myeloproliferative neoplasms (MPN) are clonal diseases originated from a hematopoietic stem cell characterized by proliferation of mature cells of one or more myeloid lineages and presenting a risk of progression to acute leukemia or bone marrow failure due to the appearance of myelofibrosis, during the evolution of the disease. In 2005, the JAK2V617F mutation was described in 95% of Polycythemia Vera (PV) and 60% of Essential Thrombocythemia (ET) and Primary Myelofibrosis (PMF) patients. Later, other molecular markers involved in cell signaling pathways (LNK, SOCS, CBL), epigenetic processes (TET2, ASXL1, IDH1, IDH2, EZH2) and in the splicing machinery of messenger RNA (SF3B1, SRSF2, U2AF1) were described. In general, these markers are found in MPN in low percentages and none of them is specific for these diseases. More recently, in late 2013, the presence of mutations in the CALR gene were reported in 49-84% of ET and 67-88% of PMF JAK2V617F negative, becoming the second most frequent molecular marker in MPN. Since its discovery, the JAK2V617F mutation has been a criterion for the diagnosis of classical MPN (PV, ET, PMF). Given its high frequency, mutations in CALR have already been proposed as a diagnostic criteria to be included in the next guide of the MPN WHO’s classification, while the diagnostic relevance of other markers is more limited. However, several studies have reported that the presence of additional alterations to JAK2 or CALR may have a role in the progression of ET and PV to MF or LAM. The main objective of this work was to identify whether the detection of a clonal hematopoiesis may have a role in the transformation of a MPN. The first two works were based on the study of a cohort of patients, mainly with JAK2V617F negative ET. Results showed that the presence of alterations in new molecular markers as TET2, ASXL1, IDH1/2 and c-CBL are infrequent in this group of patients. In addition, it was also observed that the detection of clonal hematopoiesis, by analyzing the X chromosome inactivation pattern (analyzing the HUMARA gene polymorphism), in JAK2V617F negative ET, is associated with an increased risk of myelofibrotic transformation. Moreover, although results were not statistically significant, a tendency to association between the presence of additional alterations to JAK2V617F and transformation of the disease was also observed. In the third study, a mutational study of multiple genes was performed in a cohort of patients with post-thrombocythemic myelofibrosis (post-ET MF) and post-polycythemic myelofibrosis (post-PV MF) and compared with that of patients with chronic phase MPN. Results showed that the presence of mutations in the spliceosome genes are uncommon in ET and PV in chronic phase, and that it exists a different mutational profile in the splicing machinery genes between PMF patients and post-ET MF or post-PV MF patients. Similarly, in the group of ET and PV patients who evolved to MF, statistical significance between the presence of additional mutations to JAK2 and the risk of myelofibrotic transformation could not be demonstrated. However, it was noted that the incidence of mutations in patients with secondary myelofibrosis was higher than in patients with ET or PV in chronic phase, accordingly to what has been previously described.
