Aspectos neurobiológicos de la depresión en pacientes consumidores de cocaína

Abstract

Objetivo: El objetivo general es determinar si existen diferencias neurobiológicas entre la depresión mayor primaria (DMP) y la depresión mayor inducida (DI) por cocaína en pacientes con Trastorno por Dependencia de cocaína. Las posibles diferencias se estudiarán a través del estudio de la respuesta al estrés y la implicación del sistema serotonérgico. Diseño: estudio experimental aleatorizado, doble-ciego, con tres grupos de pacientes y un grupo de controles sanos. Sujetos: 54 sujetos: 20 controles sanos sin diagnóstico psiquiátrico, 15 pacientes diagnosticados de Trastorno por dependencia de cocaína y depresión mayor primaria (DSM-IV-TR), 9 pacientes diagnosticados de Trastorno por Dependencia de cocaína y depresión inducida por cocaína (DSM-IV-TR) y 10 pacientes diagnosticados de Trastorno por Dependencia de cocaína (DSM-IV-TR). Instrumentos y métodos: Selección: Diagnóstico psiquiátrico mediante PRISM, HRDS (depresión), escala de acontecimientos vitales estresantes (LSC-R), escala impulsividad de Barrat (BIS-II), STAI-E-R. Durante las sesiones experimentales: test de ansiedad-estado STAI-E y escalas analógicas visuales (tristeza, ansiedad, alerta, felicidad, molestias gástricas y mareo). Test funcionales: Test de estrés Trier Social (TTS) y Test de Depleción Aguda de Triptófano (No-TDA/TDA). Variables: - Bioquímicas: Concentraciones de cortisol y alfa-amilasa pancreática, concentraciones plasmáticas de BDNF y concentraciones plasmáticas de triptófano (ratio de las concentraciones de triptófano y aminoácidos competidores por atravesar la barrera hetamoencefálica), - Fisiológicas: frecuencia cardíaca (FC), presión arterial diástólica y sistólica (PAD y PAS), frecuencia respiratoria (FR) y temperatura (TEM). - Clínicas: puntuaciones estado ansiedad, y puntuaciones en las escalas analógicas (tristeza, ansiedad, alerta, felicidad, molestias gástricas y mareo). Resultados: Los controles mostraron diferencias significativas en las variables: FC, PAS, PAD, FR (p<0,001), TEM (p=0,001), cortisol (p=0,004) y STAI-E (p<0,001) entre el tiempo basal (T0=’) y el inmediatamente posterior al TTS No-TDA (T=13’). Existiendo diferencias también, en casi todas las variables en la condición TTS en TDA. Con lo que se validó el Test de estrés Trier social (TTS). Todos los grupos de pacientes no mostraron diferencias significativas intragrupo en la variable cortisol entre el tiempo basal (T0=’) y el inmediatamente posterior a la prueba de estrés (T=13’) ni tampoco mostraron diferencias en sus AUCs (0’-43’), correspondientes al período de cambio más agudo en la prueba de estrés, en cada una de las variables estudiadas en cada una de las condiciones. Existiendo diferencias intergrupos entre controles y pacientes COC+DMP; y entre controles y pacientes COC, en los AUCs de la FC en las dos condiciones TTS No-TDA (p=0,011) y TDA (P=0,014). También, se observaron diferencias entre los controles y los pacientes COC+DI en el TTS en TDA en la escala analógica visual tristeza EAV-TR (p=0,038). En todos los grupos estudiados (controles y pacientes) la prueba diferencia de medias T de student entre los AUCs para cada una de la diferentes variables condición TTS No-TDA vs TDA no muestra diferencias significativas excepto en alguna variable psicológica (sobre todo en controles). Conclusiones: El TTS permite estudiar la respuesta al estrés en humanos; existiendo una activación de la respuesta del estrés a través de la activación bioquímica, fisiológica y psicológica en las dos condiciones de estrés en la mayoría de las variables en los controles sanos. Los tres grupos de pacientes muestran una respuesta aplanada al cortisol tras TTS y no hay diferencias entre ellos en el resto de variables relacionadas con el estrés. Hay diferencias en los AUC 0’-43’ en las dos condiciones del TTS en la variable FC entre el grupo control y COC y entre el grupo control y COC+DMP. En cuanto a la implicación del sistema serotonérgico: El Test de no depleción/depleción aguda de triptófano (No-TDA/TDA) no afecta a la respuesta del estrés en casi ninguna de las variables.


Aim: the aim is to determine if there are neurobiological differences between primary major depression (PMD) and induced major depression (ID) for cocaine in patients with cocaine dependence. Possible differences will be explored through the study of the stress response and the involvement of the serotonergic system. Design: randomized and double-blind experimental study with three groups of patients and one group of healthy controls. Subjects: 54 subjects: 20 healthy controls without psychiatric diagnosis, 15 patients diagnosed with cocaine dependence and primary major depression (COC+PMD) (DSM-IV-TR), 9 patients with cocaine dependence and cocaine-induced depression (COC+ID) (DSM IV-TR) and 10 patients diagnosed with cocaine dependence (COC) (DSM-IV-TR). Materials and methods: Selection: Psychiatric diagnosis by PRISM, HRDS (depression), scale of stressful life events (LSC-R), Barrat Impulsiveness Scale (BIS-II), STAI-S-R. During the experimental sessions: test anxiety-state STAI-S and visual analog scales (VAS: sadness, anxiety, alert, happy, upset stomach, and dizziness). Functional Test: Trier Social Stress Test (TSST) and Acute Tryptophan Depletion test (Non-ATD/ATD). Variables: - Biochemical: cortisol and pancreatic alpha-amylase concentrations, BDNF and tryptophan plasma concentrations (ratio of the concentrations of tryptophan and competing amino acids to cross the blood-Brain-barrier). - Physiological: heart rate (HR), diastolic and systolic blood pressure (DBP and SBP), respiratory rate (RR) and temperature (TEM). - Clinics: anxiety state scores, and visual analog scales scores (VAS sadness, anxiety, alert, happyness, upset stomach, and dizziness). Results: Controls showed significant differences in the following variables: HR, SBP, DBP, RR (p <0.001), TEM (p = 0.001), cortisol (p = 0.004) and STAI-S (p <0.001) between baseline time (T =0 ') and immediately after the TSST Non-ADT (T = 13'). Differences also exist in almost all variables in the TTS and ATD condition. So, The Trier Social Stress Test (TSST) was validated. All patient groups showed no significant intragroup differences in cortisol variable between the basal time (T0 =0 ') and the time immediately post-stress test (T = 13') nor showed differences in their AUCs (0'-43 '), corresponding to the period of acute change in the stress test, in each of the variables studied in each of the conditions. Intergroup differences exist between controls and patients COC + PMD; and between healthy controls and COC patients in yours AUCs of the HR in the two TSST Non-ATD (p = 0.011) and ATD (P = 0.014) conditions. Also, differences between the healthy controls and the COC + DI patients in the TSST in ATD in visual analogical sadness scale (p = 0.038) were observed. In all groups studied (healthy controls and patients) test mean difference between T student AUCs for each of the different variables condition TSST vs Non-ATD/ATD does not show significant differences except in some psychological variable (especially in healthy controls). Conclusions: The TSST allows to study the stress response in humans; having an activation of the stress response through biochemical, physiological and psychological activation in two conditions of stress in the most variables in healthy controls. The three groups of patients show a flattened cortisol response after TTS and no differences between them in other stress-related variables. There are differences in the 0'-43 'AUC in the two conditions (TSST Non-ATD/ATD) in the variable HR between the control group and between the COC and COC + PMD and control group. Regarding the involvement of the serotonergic system: no depletion/acute tryptophan depletion test (Non-ATD /ATD) does not affect the stress response in almost any of the variables.
