Carga tumoral total en el ganglio centinela de pacientes con cáncer de mama en estadios iniciales

Abstract

En esta Tesis Doctoral, el doctorando estudia el papel que pueden tener las técnicas moleculares recientemente introducidas en el análisis del ganglio centinela en el cáncer de mama, con el fin de poder mejorar la información que aporta éste por sí solo. De este modo, y fruto del estudio multicéntrico del que el doctorando fue Investigador Personal y que supone la base de esta Tesis Doctoral, demostró que la cantidad de tumor presente en los ganglios centinela (expresada en ARN mensajero de citoqueratina) es capaz de predecir la afectación de más ganglios, aportando una información adicional al mero hecho de decir si el ganglio analizado es positivo o no. Estos datos proporcionan una nueva herramienta a los diferentes especialistas implicados en el tratamiento de las pacientes con cáncer de mama que permite individualizar más la conducta a seguir. En el primer trabajo se estudia el valor predictivo del análisis molecular del ganglio centinela como valor predictivo de afectación de más ganglios axilares. Para ello se diseñó un estudio multicéntrico retrospectivo que reclutó a 697 pacientes. En todas ellas se determinó la Carga Tumoral Total del ganglio centinela (definida como la suma de copias de ARNm de CK19/µL, obtenida mediante el anàlisis molecular de la muestra) y se relacionó con la afectación de ganglios linfáticos no-centinela. El estudio uni y multivariado demostró que se trataba de un valor altamente relacionado con la afectación de más ganglios, por lo que podría emplearse en la práctica clínica para conocer el riesgo individual de cada paciente. Este trabajo fue publicado en la revista Breast Cancer Research and Treatment siendo el doctorando el primer autor del mismo. Tomando como base los resultados del primer trabajo se decidió construir un nomograma predictivo de afectación axilar que incorporase la Carga Tumoral Total junto con el resto de variables que mostraron tener una correlación estadística con la afectación de ganglios no-centinela. Para ello se tuvo que validar el nomograma derivado del trabajo preliminar, por lo que fue necesario una segunda población de pacientes diferente a la anterior. Se realizó por tanto un nuevo estudio multicéntrico que reunió a 436 nuevas pacientes y confirmó los hallazgos anteriormente descritos. El trabajo fue de nuevo publicado en la revista Breast Cancer Research and Treatment siendo el doctorando el último firmante del mismo. En resumen, esta Tesis Doctoral da un paso más allá en la información que aporta el ganglio centinela, a la vez que abre nuevas vías de investigación que ya han comenzado (en concreto conocer el valor pronóstico de esta nueva herramienta). En la época actual, en la que el papel del ganglio centinela está sufriendo un profundo debate, estas nuevas aportaciones ayudan a definir mejor su rol. De hecho, en los últimos meses se han ido extendiendo, tanto a nivel nacional como internacional, los trabajos que siguen con la idea de la Carga Tumoral Total como nueva herramienta para el tratamiento de las pacientes con cáncer de mama. A su vez, múltiples centros sanitarios españoles han adoptado ya las conclusiones de los trabajos presentados en esta Tesis Doctoral en su práctica diaria.


In this thesis, the student studies the role that may have recently introduced molecular techniques in the analysis of sentinel lymph node in breast cancer, in order to improve the information provided by this alone. Thus, and as a result of the retrospective multicentric study conducted by the researcher and which is the basis of this thesis, he showed that the amount of tumor present in the sentinel nodes (expressed in mRNA of cytokeratin 19) is able to predict the involvement of more nodes, providing additional information tan just saying if the node is positive or not. These data provide a new tool to clinicians involved in the treatment of breast cancer patients allowing to individualize more. In the first study, the predictive value of sentinel lymph node to predict non-sentinel lymph nodes involvement is studied. For this purpose a multicentric retrospective study that enrolled 697 patients was designed. In all of them, the Total Tumor Load (defined as the amount of CK19 mRNA copies/µL, obtained by molecular analysis of the node) was correlated with non-sentinel lymph nodes metastasis. The uni and multivariate study showed that it was highly related to the involvement of more lymph nodes, so it could be used in clinical practice to know the individual risk of each patient. This work was published in Breast Cancer Research and Treatment. Based on the results of the first work, the same group decided to build a nomogram predicting the axillary involvement that incorporated Total Tumor Burden along with the rest of variables shown to have a statistical correlation with the involvement of non-sentinel lymph nodes. To do so, they had to validate the nomogram derived from the preliminary work so a second population of patients, different from the first one was necessary. A new multicentric study gathered 436 new patients and confirmed the findings described in the first study. This work was published again in Breast Cancer Research and Treatment. In summary, this thesis takes a step beyond the information provided by the sentinel node, while opening new ways of research that have already started (i.e. to determine the prognostic value of this new tool). In the current era, in which the role of sentinel lymph node is undergoing a deep debate, these new contributions help to better define its role. In fact, in the last months, many papers have been published that follow with the idea of Total Tumor Load as a new tool for the treatment of breast cáncer patients. Furthermore, many Spanish institutionshave already adopted the conclusions of the work presented in this thesis in their daily practice.
