Interès de la utilització d'una escala de risc en la decisió d'hospitalització dels pacients amb pneumònia comunitària

Abstract

Introducció: La pneumònia comunitària (PC) constitueix una de les patologies infeccioses més prevalents. La decisió d'hospitalització dels pacients amb PC és un acte lligat a la subjectivitat dels metges d'urgències. En la PC, s'ha vist que els costos finals associats al seu tractament es multipliquen per 20 quan el pacient és ingressat. Malgrat tots aquests coneixements, mai s'han avaluat les implicacions de la utilització d'una escala en la decisió d'ingrés o alta hospitalària del pacient afectat de PC. La nostra hipòtesi ha sigut que l'aplicació de l'escala de Fine (PSI) en la decisió d'ingrés o alta hospitalària del pacient afectat de PC en els Serveis d'Urgències Hospitalaris (SUH) permetria adequar millor el circuit assistencial d'aquests pacients.<br/>Metodologia: Tipus d'estudi: estudi clínic observacional descriptiu, prospectiu, multicèntric, no aleatoritzat. Àmbit de l'estudi: nou hospitals de la xarxa de la Sanitat Pública francesa. Població de l'estudi: (1) Unitats estudiades: activitat dels centres, agrupats en dues classes: centres A (utilitzen de rutina el PSI en la decisió d'hospitalització del pacient amb PC) i centres B (no utilitzen aquesta escala en la decisió d'hospitalització del pacient amb PC). (2) Criteris d'inclusió: pacients majors d'edat visitats en els SUH diagnosticats de PC i que haguessin donat el consentiment informat per participar en l'estudi. Criteris de no inclusió: infecció pel V.I.H., immunodepressió, portadors de traqueotomia o ventilació mecànica crònica. Variables resultats: Estat clínic a 28 dies, percentatge d'hospitalitzacions, percentatge de reingressos, costos totals. Desenvolupament de l'estudi: recollida de dades a dia zero, control microbiològic a dia 15 i un control clínic i de consum de recursos a dia 28. L'anàlisi estadístic va ser realitzat segons el model de "intention to treat". Tots els anàlisis els vàrem fer amb una aproximació bilateral. L'estudi va ser aprovat pel Comitè d'Ètica de l'hospital Henri Mondor.<br/>Resultats: 670 pacients foren inclosos. Les característiques basals dels pacients dels dos grups foren similars, la distribució segons el PSI fou: 18% classe I, 13% classe II, 17% classe III, 35% classe IV i el 17% classe V. En total, 536 pacients (79% als centres A, 81% als centres B, p=0.498) foren ingressats al moment del diagnòstic i 134 (20%) foren tractats de manera ambulatòria. El 46% dels pacients de les classes I i II varen ser hospitalitzats (37% als centres A, 57% als centres B, p=0.003), d'aquests, el 31% ho foren basant-se en criteris subjectius (2% als centres A, 7% als centres B, p=0.000). No detectàrem diferències significatives en l'estat clínic a dia 28 sobre els dos tipus de centres. La despesa mitjana associada al tractament de la PC fou de 7126.83 per pacient hospitalitzat i de 490.05 per pacient tractat en manera ambulatòria. Al centrar l'anàlisi en funció de cada classe de Fine, detectàrem que en els pacients classes I i II es produí un estalvi mitjà de 2062.56 per pacient tractat segons l'algoritme de Fine.<br/>Conclusions.<br/>(1) L'aplicació del PSI en la decisió d'orientació del pacient amb PC atès als SUH, és una eina segura en quant a resultats clínics. (2) L'aplicació del PSI en la decisió d'orientació del pacient amb PC atès als SUH, permet de reduir els ingressos hospitalaris dels pacients de les classes I i II en un 20%. (3) L'aplicació del PSI en la decisió d'orientació del pacient amb PC atès als SUH, permet d'obtenir un estalvi de costos mitjà de 455.37 /pacient. (4) Per a la implementació del PSI en la decisió d'orientació del pacient amb PC atès als SUH, detectem la necessitat de sistemes de suport que facilitin el seguiment de les recomanacions reflectides en l'escala.


INTRODUCTION: <br/>Community Acquired Pneumonia(CAP) is one of the most prevalent infectious pathologies. The decision to hospitalize a patient with CP is based on the subjectivism of the doctors in the emergency room. Observations lead to the conclusion that the costs of treatment result 20 times greater when a patient with CAP is hospitalized. Despite these observations, a standardised evaluation of each case has not yet been applied. This would facilitate the decision on hospitalizing a patient or not. Our hypothesis states that the use of the scale of Fine(PSI) would allow a better and more efficient assistance in the Emergency Department (ED) to the affected patients.<br/>METHODOLOGY: <br/>Type of Study: descriptive observatory clinical study, prospective, multi-centre not randomised. <br/>Research area: Nine hospitals within the net of the French Public Sanitary Assistance. <br/>Research Population : <br/>(1) The studied units are: Activities of the centres, grouped in two categories:<br/>A -group using the routine based on the PSI- Scale and <br/>B - group not using that scale when comes to make the decision to hospitalize the patient suffering CAP. <br/>(2) Criteria of inclusion: older patients checked at the Emergency Unit and found suffering from CAP as well as accepting to participate in this research study. <br/>Results units: Clinical results after 28 days, percentage of the patients' admissions , percentage of patients re-admitted, and total costs. <br/>Development of the study: Register of the data the day zero (starting day of study), microbiological control at the day 15th , and a clinic control and control in the use of resources at the day 28th. The statistical analysis has been made according with the model "intention to treat". All the analysis were done by a bilateral rough guess. The study was approved by the Ethic Committee of the Henri Mondor Hospital. <br/>RESULTS:<br/>670 patients have been included in this study. The basic characteristics of the patients of the two groups were similar., the distribution according to the PSI was.: 18 % class I, 13 % class II, 17 % class II, 35 % class IV and 17 % class V. In total , 536 patients (79 % all the centres A , 81 % all centres B, p= 0,498) were hospitalized the same moment they were given a diagnostic, and 134 (20 % ) were treated ambulatory. The 46% of the classes I and II were hospitalized (37 % to the centres A and 57 % to the centres B, p= 0.003) , from those the 31 % were qualified based on the doctors' subjectivity (2 % in the centres A and 7 % in the centres B, p=0.000). By the day 28 there were no significant differences detected in the clinical stage between both centres. The total average costs associated to the treatment of the CP were from 7126.83 per hospitalized patient and from 490.05 per patient treated ambulatory. While making the analysis based on every level of Fine, we could find that 2062.56 per patient were saved thanks to the use of the algorithm of Fine System. <br/>CONCLUSIONS<br/>(1) The application of the PSI at the moment deciding about a patient with CP who has come to the ED , it has shown to be a great tool to assure the clinical results.<br/>(2) The application of the PSI in the case of patients with CP coming into the ED allows to reduce 20 % of both class I and II that have to be hospitalized. <br/>(3) The application of the PSI for orientation in the decision about a patient with CP entering the ED facilitates a saving of costs reaching 455.37 per patient.<br/>(4) To implement the PSI , making decisions in the ED about patients with CP it is needed some sort of follow up support system regarding the recommendations reflected in the scale.
