Abstract

Introducció:<br/>La gonartrosi, o artrosi del genoll, és la causa més freqüent de dolor al genoll després dels 50 anys. Es calcula que la gonartrosi causa símptomes invalidants en un 10% de les persones más grans de 55 anys i que una cuarta part d'aquestes pateixen una discapacitat greu.<br/>El tractament d'elecció de la gonartrosi invalidant és l'artroplàstia total de genoll, tècnica eficaç i cost-efectiva.<br/>Conèixer les expectatives del pacient de trobar millora amb la intervenció quirúrgica és important perquè com més s'apropin els resultats obtinguts d'un acte mèdic a les expectatives prèvies, més probabilitat d'obtenir la satisfacció del pacient.<br/>L'objectiu d'aquest treball és aprofundir en l'estudi de les expectatives prèvies a la intervenció d'artroplàstia total de genoll, en pacients diagnosticats de gonartrosi.<br/>Material i Mètode:<br/>Estudi transversal<br/>Criteris d'inclusió: <br/>- diagnòstic de gonartrosi i en llista d'espera per a ATG<br/>- sense trastorn cognitiu que interferís la comprensió dels qüestionaris<br/>- sense dificultat idiomàtica per a comprendre els qüestionaris<br/>Criteris d'exclusió:<br/>- pacients sense dades d'expectatives<br/>- pacients en espera d'ATG de revisió<br/>- pacients en espera d'artroplàstia unicompartimental<br/>Variables recollides : Demogràfiques, Funció, Dolor, Estat de Salut, Estat Emocional, Avaluació Cognitiva, Expectatives.<br/>Resultats:<br/>La sèrie final de pacients que cumplian criteris es va composar de 497 pacients: 375 dones i 122 homes; amb una edat mitjana de 72.0 anys (DE7.0); dels quals 126 vivien sols i 371 amb la família; 363 referien estudis primaris, 108 eren analfabets i 15 tenien estudis superiors; 105 van consultar amb el Treballador Social abans de l'ingrés hospitalari i 198 manifestaven tenir dificultats d'accés al domicili habitual. El conjunt de la sèrie es trobava dins del rang de l'obesitat. Els pacients, en general no presentaven limitacions per a les AVD bàsiques, tenien pocs símptomes depressius, baixa comorbiditat, poc deteriorament cognitiu, un dolor al genoll abans de la intervenció entre moderat i sever, i la funció del genoll mitjanament limitada. Els dominis més alterats del SF-36 corresponien a la Funció Física i al Dolor corporal.<br/>Els valors trobats de les expectatives van ser: 12.3 (DE 1.6) per a les Expectatives Totals i 9.5 (DE 1.7) per a les Expectatives Màximes. Les Expectatives més valorades van ser : millora del dolor, millora del benestar general, millora en la deambulació, en pujar i baixar escales, en la mobilitat del genoll i en la mobilitat en general. <br/>Es van trobar correlacions estadísticament significatives entre les Expectatives i variables demogràfiques (edat, sexe, consulta amb el treballador social, portar una ATG prèviament); de funció (índex de Barthel i component resum Físic del SF-36); de dolor (dolor esperat als 6 mesos de la intervenció); d'estat emocional (símptomes depressius mesurats amb la GDS de Yesavage) i estat cognitiu.<br/>Segons l'anàlisi de regressió lineal les variables explicatives de les Expectatives Totals foren : l'índex de Barthel, edat i valor de la GDS mentre que de les Expectatives Màximes eren l'índex de Barthel, edat i valor de la GDS i s'afegien el sexe, dolor mesurat amb l'EVD i dolor esperat als sis mesos de la intervenció.<br/>Es van confeccionar taules de valors normals d'Expectatives Totals i Màximes.<br/>Conclusions:<br/>1. Les expectatives de millora són elevades<br/>2. Que aquestes es poden agrupar en: millora del dolor, millora d'aspectes funcionals bàsics i millora del benestar general<br/>3. S'han establert correlacions entre les expectatives i variables clíniques i demogràfiques<br/>4. Els resultats d'aquest estudi han permès confeccionar taules amb valors de normalitat per a la població de referència


Background: <br/>Knee osteoarthritis is the most frequent cause of knee pain in subjects higher of 50 years old. It has been estimated that in 10% of people over 55 years present disabling symptoms caused by knee osteoarthritis and in a quarter of the cases, disability is severe. The treatment of choice in disabling knee osteoarthritis is total knee arthroplasty (TKA), a technique cost-effective. To have knowledge of patients' expectations concerning surgery results is very important because the more similar the outcomes achieved to previous expectations are, the higher probability to get patients satisfied. The main objective of this study was to go more deeply in the study of expectations of patients with knee osteoarthritis have before surgery. <br/>Patients and Method: <br/>Transversal study of 497 consecutive patients.<br/>Inclusion criteria were:<br/>- Patients with knee osteoarthritis waiting for intervention of TKA<br/>- Absence of cognitive impairment that could interfere the fullfilling of questionnaires<br/>- Absence of language difficulties to understand questionnaires<br/>Patients were excluded if:<br/>- They were waiting for revision of TKA or unicompartimental arthroplasty<br/>- They not had data available regarding expectations<br/>The main variables collected were:<br/>- Demographic (age, gender, studies and family status) <br/>- Expectations Scale for TKA from the Hospital for Special Surgery (17 items)<br/>- A shortened version of the Geriatric Depression Scale (GDS)<br/>- Health-related quality of life assessed with the Short Form 36 (SF-36)<br/>- Charlson comorbidity index<br/>- Pfeiffer Short Portable Mental Status Questionnaire (SPMSQ)<br/>- Pain and function subscales of the Knee Society Clinical Rating Scale (KSS)<br/>- Verbal Analogue Scale (VAS) of Pain<br/>- Barthel Index<br/>- History of previous TKA.<br/>Results: <br/>122 men and 375 women with a mean age of 72.0 (SD 7.0). 371 lived with other family members and 126 lived alone. Educational level was primary studies in 363 cases, secondary and high studies in 15, and 108 can not read or write. 105 subjects had contacted to social worker before hospital admission and 198 referred architectural barriers to access to their houses. Mean weight of the sample was within the range of obesity. Most of the patients were able to perform the activities of daily life. The sample did not present depressive symptoms, comorbidity or cognitive impairment either. They referred painful knee of moderate-high intensity and knee function was also moderately limited. The most affected dimensions in the SF-36 were Physical function and Body pain.<br/>The values of total and maximal expectations were 12.3 (SD 1.6) and 9.5 (SD 1.7), respectively. The most appreciated expectations were those concerning improvement in pain, general well-being, walking, going up and down the stairs, mobility of the knee and general mobility.<br/>A significant statistically correlation was found between expectations and demographic variables (age, gender, social worker intervention, being carrier of a total knee replacement in the contralateral limb), functional variables (Barthel Index and the Physical Standardised Component of the SF-36), pain (pain expected at 6 months after surgery), emotional variables (GDS) and cognition.<br/>A linear regression analysis showed that explanatory variables of the total expectations were age, Barthel Index and the GDS. The same analysis for the maximal expectations pointed at age, gender, Barthel Index, pain assessed with the VAS of pain and pain expected at 6 months after surgery as the explanatory variables.<br/>The tables of normality of both total and maximal expectations were constructed. <br/>Conclusions: <br/>- The expectations of improvement in patients before undergoing TKA are high<br/>- The most appreciated expectations by patients might be grouped in pain relief, basic functions (walking, stairs, knee range of motion, global mobility) and well-being.<br/>- A significant correlation of expectations with clinical and demographic variables is established<br/>- The results of this study have allowed constructing tables of normality for the reference population.
